范永升教授诊治自身免疫性肝炎心得

摘　　要：自身免疫性肝炎是累及肝实质的器官特异性自身免疫病,范永升教授认为本病多因肝经湿热瘀毒所致,其基本病机为肝气郁结,气滞血瘀;脾胃失调,湿热内壅;热毒伤阴,肝肾阴亏。本病属本虚标实,病机关键为肝络郁滞,即湿、热、瘀、毒是本病之原,阴阳气血虚弱是本病之本,肝络郁滞是病机的中间环节。病位在肝胆脾胃,治法重在疏利肝经湿热,活血祛瘀解毒。临证时范教授善重用赤芍、虎杖、白芍、炒枳壳、淮小麦,取效明显。Autoimmune hepatitis （AIH） is a specific autoimmune disease involved liver parenchyma organ, Professor FAN Yong-sheng considers that it is caused by damp-heat, blood stasis and pathogenic toxin of liver channel, its basic pathogenesis is liver-qi stagnation, qi stagnation and blood stasis; spleen and stomach disorders, damp-heat congestion; toxic heat damaging yin, deficiency of liver and kidney. This disease is asthenia in origin and asthenia in superficiality, the key pathogenesis is stagnation of liver meridian, wet, heat, blood stasis, poison is the cause of the disease, insufficiency of vital energy and blood, deficiency of yin and yang is the root of it, the liver channel stasis is the intermediate link of its pathogenesis. Its lesion site is in liver, gallbladder, spleen and stomach. Treatment focuses on smoothing damp-heat in liver channel, activating blood stasis and detoxicating. Professor FAN specializes in heavy use of red paeonia, polygonum cuspidatum, radix paeoniae alba, fried bitter orange, huai wheat in clinical. He has a wealth of clinical experience and unique effect in the treatment of AIH.
