摘　　要：目的了解维持性血液透析患者钙磷代谢现状，干预后疗效。方法测定2010年7月到2010年12月沈阳医学院沈洲医院门诊维持血液透析的慢性肾衰竭患者血清钙、磷、iPTH，对未达标患者进行药物干预一年，对患者治疗前后的症状和化验值进行比较。结果①治疗后患者的临床症状改善明显，治疗前后比较有统计学意义。②治疗后患者血钙上升，iPTH、血磷及钙磷乘积降低，于治疗前相比有统计学意义。③治疗后患者血清甲状旁腺素水平、钙、磷及钙磷乘积的达标率分别为80％、63，6％、61％、84％，于治疗前相比达标率明显上升，具有统计学意义。结论慢性维持性血液透析患者普遍存在矿物质及骨代谢紊乱，在K／DOQI指南指导下对患者进行药物干预治疗，能够改善患者钙磷代谢异常，提高达标率，缓解临床症状。Objective To investigate the calcium and phosphorus metabolism status and the efficacy after intervention in maintenance hemodialysis patients.Methods Serum calcium, phosphorus and iPTH levels were determined in Shenyang Medical College Shenzhou Hospital outpatient maintenance hemodialysis patients with chronic renal failure fi＇om July 2010 to December 2010, and the patients did not meet the criteria were given drug intervention for 1 year. Symptoms and laboratory values were compared before and after treatment. Results OThe patient＇ s clinical symptoms improved after treatment. （2）Serum calcium increased and iPI＇H, phosphorus, calcium and phosphate product decreased after treatment, they are all statistically significant compared to before treatment state. （3）Serum level of parathyroid hormone, calcium, phosphorus, and calcium - phosphate product compliance rate was 80%, 63.6%, 61%, 84% after treatment compliance rate was significantly increased compared to before treatment state, with statistical significance. Conehtsions The prevalence of mineral and hone metabolism disorders exists in patients with chronic maintenance hemodialysis, and drug intervention can improve the patients with abnormal calcium and phosphorus metabolism, increase the compliance rate, the clinical symptoms of patients under the guidance of the K/DOQI guidelines.
