摘  要:目的观察氟伐他汀对扩张型心肌病心功能与血浆脑钠肽水平的影响。方法选择86例扩张型心肌病患者作为研究对象,利用随机数字表法将其分为两组。对照组43例给予常规药物治疗;研究组43例在常规治疗的基础上给予氟伐他汀治疗。观察两组患者的心功能相关指标和血浆脑钠肽水平的变化。结果研究组治疗的总有效率为90.7%,显著高于对照组的67.4%,两者比较,差异有统计学意义(P<0.05)。治疗后,研究组患者的心功能NYHA分级等级明显降低(P<0.05);左心室射血分数(LVEF)、每搏量(SV)、心脏指数(CI)均得到明显的改善(P<0.05);左心室舒张末内径(LVEDd)有缩小的趋势,但较治疗前无统计学差异(P>0.05)。对照组各项指标也有改善,但较治疗前均无统计学差异(P>0.05)。治疗后,两组患者的血浆脑纳肽(BNP)水平均明显降低(P<0.05),且研究组明显低于对照组(P<0.05)。两组患者在治疗过程中均未出现不良反应。结论氟伐他汀能够有效地改善扩张型心肌病患者的心功能,安全性高。Objective To study the clinically therapeutic effect of Fluvastatin on dilated cardiomyopathy (DCM) and the influence on heart function and brain natriuretie peptide (BNP) level. Methods Total of 86 DCM patients were divided into trial group and control group (n = 43/group) according to the random number table. The control group was treated by conventional medical treat- ment and the trial group was additionally treated by Fluvastatin. Clinical curative effect and changes in heart function and plasma BNP level were observed. Results The total effective rate of the trial group was 90.7% , significantly higher than that ( 67.4% ) of the control group, the difference has statistical significance ( P 〈 0.05 ). After treatment, the heart function level by NYHA clas- sification of the trial group obviously decreased when compared with before treatment ( P 〈 0.05 ) , and stroke volume ( SV), left ventricular ejection fraction (LVEF) and cardiac index (CI) obviously improved ( P 〈 0.05 ). Although left ventricular end-dias- tolic diameter (LVEDd) of the trial group reduced to some extent, there was no significantdifference from before (P 〉 0.05 ). Each index of the control group was improved after treatment, but it was of no significant difference from before treatment (P 〉 0.05). BNP level of the trial group obviously reduced after treatment ( P 〈 O. 05 ) , and was obviously lower than that of the control group ( P 〈 0.05 ). In the treatment process, all the patients did not appear to suffer from adverse reaction. Conclusion Fluvastatin ap- plied to DCM can effectively adjust heart funetion, and has a high safety.
