髓内钉固定植骨与钢板内固定植骨治疗胫骨骨不连的临床对比研究

摘　　要：目的探讨髓内钉固定植骨与钢板内固定植骨治疗胫骨骨不连的临床疗效。方法回顾性分析胫骨骨不连患者的临床资料并分为接受髓内钉固定植骨治疗的观察组和钢板内固定植骨治疗的对照组,观察术后骨折断端愈合情况、负重下地时间以及膝关节功能。结果观察组影像学愈合例数、时间以及完全负重下地时间均明显短于对照组,HSS总评分、功能评分、活动度评分、肌力评分、稳定度评分均明显高于对照组。结论髓内钉内固定植骨治疗能够促进骨折断端的愈合和膝关节功能的恢复,具有积极的临床价值。Objective To study the therapeutic effects of intramedullary nail fixation plus bone grafting and plate fixation plus bone grafting on tibial nonunion. Methods Clinical data of patients with tibia nonunion, who underwent intramedullary nail fixation plus bone grafting （trial group） and plate internal fixation plus bone grafting （control group） , were retrospectively studied, including fracture healing, weight-bearing time and knee joint function. Results The parameters, such as radiographic healing case number, healing time and full weight-bearing time were better in the trial group than in the control group, and so were total HSS （ hospital for special surgery） knee score, functional score, activity score, muscle strength score and stability score. Conclusion The combina- tion of intramedullary nail internal fixation and bone grafting is of positively clinical value, for it can promote the healing of fracture and the recovery of knee joint function.
