摘  要:目的 探讨腹腔液中白细胞介素-6(IL-6)、转化生长因子-β(TGF-β)和基质金属蛋白酶-7(MMP-7)在不同r-AFS分期的子宫内膜异位症(EMs)患者腹腔液中的表达。方法 收集80例行腹腔镜检查及治疗的患者腹腔液,其中35例为无子宫内膜异位症的对照组,36例为子宫内膜异位症Ⅰ、Ⅱ期患者,9例为子宫内膜异位症Ⅲ、Ⅳ期的患者。采用双抗体夹心酶联免疫吸附(ELISA)法检测腹腔液中IL-6、TGF-β和MMP-9的浓度,并进行相关性分析。结果 子宫内膜异位症患者腹腔液中IL-6、TGF-β和MMP-7表达分别为(736.5±526.6)ng/L、(936.5±523.5)ng/L和(5.63±1.98)μg/L,明显高于对照组,差异均有统计学意义(均P〈0.01)。子宫内膜异位症Ⅲ、Ⅳ期IL-6和MMP-7浓度分别为(948.6±485.4)ng/L、(8.69±1.23)μg/L,高于Ⅰ、Ⅱ期(453.6±256.8)ng/L、(4.23±1.08)μg/L,差异有统计学意义(均P〈0.05)。IL-6与MMP-7的表达呈显著正相关(r=0.789,P〈0.01)。TGF-β与MMP-7的表达呈显著负相关(r=-0.458,P〈0.05)。结论 子宫内膜异位症患者腹腔液中IL-6、TGF-β、MMP-7可能导致腹腔微环境免疫失衡,并对子宫内膜异位症的发生发展起一定的促进作用。Objective To evaluate the expression of IL-6,TGF-β,matrix metalloproteinase-7 (MMP-7) in the peritoneal fluid of different stage of endometriosis(EMs) patients.Methods Peritoneal fluid samples from female without EMs (n=35) and 45 patients with EMs (36 cases with stageⅠ,Ⅱ;9 cases with stage Ⅲ,Ⅳ)were obtained for detection of IL-6,TGF-β,MMP-7 by ELISA.Results The concentration of IL-6,TGF-β,MMP-7 was (736.5±526.6)ng/L,(936.5±523.5)ng/L and (5.63±1.98)μg/L in EMs patients,which were higher than those in control group (P〈0.01).The concentration of IL-6,MMP-7 was (948.6±485.4)ng/L,(8.69±1.23)μg/L in stageⅢ,Ⅳgroup,and (453.6±256.8)ng/L,(4.23±1.08)μg/L in stageⅠ,Ⅱgroup,there were significant differences between the two groups (P〈0.05).The concentration of MMP-7 was positively correlated with IL-6(r=0.789,P〈0.01),and was negatively correlated with TGF-β(r=-0.458,P〈0.05).Conclusion IL-6,TGF-β,MMP-7 may play roles in microecology immune imbalance and the development of EMs.
