摘　　要：目的了解北京市延庆县1999—2011年志贺菌群分布及耐药趋势,为细菌性痢疾防治工作提供科学依据。方法监测延庆县1999—2011年肠道门诊菌痢患者新鲜便标本,送检至辖区疾病预防控制中心分离培养出志贺菌,并进行血清分型和药敏试验。结果 13年中共检测877份便标本,分离出99株志贺菌,这99株菌分布于志贺菌属4个群的13个血清型,优势菌群是B群,占72.7%;其次为D群,占18.2%;流行优势菌型为F2a、D、F2b。B群所占比例有逐年下降趋势,D群有逐年升高趋势。药敏试验结果显示多重耐药菌株逐渐增多,实验菌株对头孢类抗生素敏感,对氨苄西林等耐药率高。结论延庆县志贺菌属随着时间推移菌群在不断变迁,菌型日渐复杂,需继续监测,摸清痢疾病原菌群的变迁规律。临床用药应结合具体情况合理选择药物,延庆县治疗菌痢应首选第3代头孢类药物。[Objective]To understand the distribution and resistance trends of dysentery flora in Yanqing County of Beijing City from 1999-2011,and provide scientific evidence for bacillary dysentery control.[Methods]The fresh stool specimens of bacillary dysentery outpatients in intestinal clinic were monitored,and submitted to CDC for Shigella isolation and culture,as well as serotyping and susceptibility testing in Yanqing County from 1999-2011.[Results] A total of 877 stool specimens were monitored during 13 years,99 strains of Shigella were isolated,belonged to 4 groups and 13 serotypes of Shigella,dominant flora was group B which occupied 72.7%,followed by group D which occupied 18.2%.The epidemic bacterial type was F2a,D,F2b.The proportion of group B was descending annually,while group D is increasing annually.Susceptibility test results showed multi-resistant strains gradually increased.The experimental strains were sensitive to cephalosporins and highly resistant to ampicillin.[Conclusion]Shigella flora is changing over time in Yanqing County.Bacteria type is increasingly complex which needs continuous monitor to find out the changes law of dysentery pathogen group.The clinical medication should be rational according to the specific conditions.Third generation cephalosporins was the first choice for treatment of bacillary dysentery in Yanqing County.
