双钢板治疗胫骨平台SchatzkerⅥ型骨折32例临床分析

摘　　要：目的探讨双钢板内固定治疗SchatzkerⅥ型骨折的临床疗效。方法对32例SchatzkerⅥ型胫骨平台骨折采用切开复位双钢板内固定治疗。于术后1、2、3、6、9、12个月复查,分别进行临床查体、X射线评估及康复指导。32例患者术后均获随访,平均随访时间为18个月。结果 X射线愈合时间14~24周,按Rasmussen评分标准进行膝关节功能评分18~29分,平均（24.2±3.3）分,其中优23例,良4例,可3例,差2例,优良率为84.4%[（优＋良）/总例数×100%]。按Rasmussen膝关节放射学评分为11~16分,平均15.1分,其中优25例,良5例,可2例,[（优＋良＋可）/总例数×100%]为93.8%。结论双钢板内固定及植骨是治疗胫骨平台SchatzkerⅥ型骨折的有效方法,术后配合康复功能训练,可获得满意的临床效果。Objective To explore the clinical efficacy of internal fixation with double plates for treating Schatzker type VI tibia] plateau fracture. Methods 32 cases of Schatzker type VI tibia] plateau fractures were treated by open reduction and in- ternal fixation with double plates. Reexamination was taken in 1,2,3,6,9,12 months after operation, and the physical examina- tions,X-ray evaluations and rehabilitation instructions were performed. Results 32 cases were followed up for average 18 months. The healing time of X-ray was 14-24 weeks and the knee function was assessed by adopting Rasmussen knee score with the scores range of 18-29 ,mean 24.2＋3.3.23 cases were excellent,4 cases were good ,3 cases were fair and 2 cases were poor, with the excellent and good rate of 84.4%. The Rasmussen knee radiologic ratings ranged ll-16,with a mean of 15.1. Among them, 25 cases were excellent, 5 cases were good and 2 cases were fair, with the excellent and good rate of 93.8%. Conclusion Internal fixation with double plates and bone graft are the effective method for treating Schatzker type VI tibial plateau fracture, which cooperated with postoperative rehabilitation can obtain satisfactory clinical effects.
