摘 要:目的 探讨体元模型动态三维重建超声显像评价肺动脉高压对右心室容积及心肌游离壁质量的影响。方法 将 2 5例各种原因引起的肺动脉高压患者 ,按疾病对右心室产生的负荷性质分为以容量负荷过重为主和以压力负荷过重为主的肺动脉高压两组 ;15例正常人为对照组。结果 15例正常人和 2 5例肺动脉高压患者右室腔及游离壁均获得满意的三维重建 ,肺动脉高压患者右室腔较正常人膨展饱满 ,右室游离壁呈瓦片状。正常人和肺动脉高压患者右室游离壁的质量分别为 (18.17± 5 .2 3)g和 (39.81± 12 .31)g ,舒张末期容积分别为(34.5 7± 11.0 1)ml和 (76 .15± 41.2 6 )ml,收缩末期容积分别为 (19.17± 6 .34)ml和 (4 6 .6 9± 2 7.86 )ml,肺动脉高压患者各项指标均大于正常人 (P <0 .0 5 )。其中压力负荷增加为主的肺动脉高压患者的右室游离壁质量(4 7.37± 11.71) g ,明显高于容量负荷增加为主的肺动脉高压患者 (31.6 2± 6 .35 ) g(P <0 .0 5 ) ,但两者的收缩末期容积、舒张末期容积、射血分数之间无明显差别 (P >0 .0 5 )。结论 肺动脉高压患者右室腔容积和游离壁质量均大于正常人 ,同时以压力负荷过重为主的肺动脉高压患者右室游离壁质量大于以容量负荷过重为主的肺动脉高压患者。但无论压力负荷过重或容量负?Objective To evaluate the effect of pulmonary hypertension on right ventricular (RV) volume and free wall mass by dynamic threedimensional voxel imaging.Methods Twentyfive patients with pulmonary hypertension were divided into two groups, Group A included patients with volume overload, Group B included patients with pressure overload.Control group included 15 normal subjects.Results Threedimensional reconstruction of the RV volumes and free wall mass was successfully performed in all the subjects. The means of free wall mass in the normal subjects and the patients were( 18.17 ± 5.23 )g and ( 39.81 ± 12.31 )g respectively. The mean end diastolic volumes of the normal people and the patients were ( 34.57 ± 11.01 )ml and ( 76.15 ± 41.26 )ml respectively. The mean end systolic volumes of them were ( 19.17 ± 6.34 )ml and ( 46.69 ± 27.86 )ml respectively. Both the RV volume and mass of the patients were larger than those of normal persons(P< 0.05 ).The RV mass of group B [( 47.37 ± 11.71 )g] was greater than that of group A [( 31.62 ± 6.35 )g,P< 0.05 ].Conclusions Both RV volume and free wall mass of pulmonary hypertension patients were greater than those of normal subjects. The free wall mass of patients with pressure overload is greater than that of patients with volume overload. However, the right ventricular volume and ejection fraction was of no significant difference between these two groups.
