摘  要:目的 探讨动脉狭窄性病变远端的超声血流动力学变化与狭窄程度的关系。方法 人工钳夹犬腹主动脉制作不同程度狭窄 (4 0 %~ 5 0 %、6 0 %~ 75 %和 80 %~ 90 %的面积狭窄 )及不同狭窄长度 (长 1cm、2cm和 3cm)的模型 ,彩色超声检测各种狭窄模型时双侧股动脉的流速曲线及血流参数。结果 狭窄程度的增加可使股动脉流速曲线反向波减小或消失 ,频带增宽 ,重度狭窄时呈单峰连续波型或静脉样流速曲线。多元线性回归得出了狭窄远端血流参数峰值流速 (PSV )、收缩期加速度 (SA)、搏动指数 (PI)和阻力指数 (RI)分别与狭窄程度和长度有统计学意义。结论 犬腹主动脉不同程度和不同长度的狭窄与股动脉的流速曲线及血流参数有一定关系 ,4种参数中以PSV。Objective To evaluate the relationship between a proximal artery stenosis and downstream Doppler waveform in dogs.Methods Five dogs were operated to make arterial stenosis models by suppressing aorta with pincers with three types of stenosis extents (40%~50%,60%~75% and 80%~90% area reduction) and there kinds of stenostic length (1 cm, 2 cm and 3 cm) in each type.The waveforms and hemodynamical parameters of bilateral femoral arteries were detected with color Doppler in each type of stenosis models including the peak systolic velocity (PSV), the systolic acceleration (SA), pulsatility index (PI) and the resist and index (RI).Results As the stenostic degree was increased gradually, the triphasic waveform of femoral artery became biphasic and monophasic waveform accordingly, and in the severe degree, it was displayed TardusParvus or continuous waveform. Linear regression analyses showed that there was statistical correlation between stenostic artery and downstream parameters (PSV, SA, PI and RI).Conclusion Quantitative evaluation of the downstream waveform parameters (PSV, SA and PI) may be useful to predict a aorta stenosis.
