隐源性机化性肺炎的CT诊断

摘　　要：目的探讨隐源性机化性肺炎（COP）的CT表现。方法经手术或活检证实的隐源性机化性肺炎31例，均行胸部螺旋CT扫描，对其CT图像作回顾性分析，分析内容包括病变部位、形态、性质、合并胸水等。治疗后疗效评价分为：治愈、好转、无变化、进展。结果病变发生于左肺5例，右肺12例，双肺同时受累14例；上叶7例，中叶2例，下叶8例，上、下叶同时受累14例。病灶表现为实变影（58．1％）、磨玻璃密度影（41．9％）、结节影（19．4％）、条带状实变影（12．9％）、肿块（6．5％）及网格状影（12．9％），以胸膜下及支气管束周围分布为主。16例治疗后随访的患者中，治愈3例、好转9例、病灶无变化1例、病变呈游走反复3例、无病情进展病例。结论CT检查对于COP的病变部位、范围及治疗后随访具有重要价值，并能监测疗效及指导用药方案。Objective To investigate the CT appearances of cryptogenic organizing pneumonia（COP）. Methods Thirty-one cases of COP confirmed by pathology were received chest spiral CT scanning examinations. The location, shape, density and whether pleural effusion or not were analyzed retrospectively. Treatment efficacy were evaluated into cured, improved, not changed, or progressed. Results Lesions located in the left lung in five cases, in the right lung in twelve cases and in both sides of the lung in fourteen cases. The upper lobe were involved in seven cases, middle lobe in two cases, lower lobe in eight cases and the both upper and lower lobes in fourteen cases. The patterns of lung abnormality were consolidations{ 58.1 ~ } , ground-glass opacifieation（ 41.9%） , nodules （19.4%） , band-like opacities （ 12.9%）, mass（ 6.5%） and reticular opacities（ 12.9% ）, and distributed mainly along the subpleural lungs or bronchovascular bundles. Of sixteen cases of patients with follow-up after treatment, there were three cases cured,nine cases improved,one case not changed,three cases migrated and no cases progressed. Conclusion CT scan can play an important role in the diagnosis of COP ,and show the abnormal changes after medical treatment.
