摘 要:目的探讨扩散加权成像(DWI)对中央区前列腺癌和前列腺增生的鉴别诊断价值。方法分析26例前列腺癌和28例良性前列腺增生的DWI和ADC图像表现,测量癌肿区和前列腺增生区的ADC值,对2组间进行比较。结果26例前列腺增生的中央腺体在DWI上呈不均匀稍高信号,2例呈明显高信号,ADC值平均为(1.13±0.18)×10^-3mm^2/s。24例前列腺癌在DWI上呈明显高信号,2例呈稍高信号;26例前列腺癌的平均ADC值为(0.89±0.18)×10^-3mm^2/s。2组ADC值间差异有统计学意义(t=-4.899,P〈0.05)。T2WI、DWI及两者结合诊断中央区前列腺癌的敏感性分别为85.7%、92.3%及95.6%。结论T2W1联合DWI在鉴别中央区前列腺癌及前列腺增生中具有重要价值。Objective To describe the appearances of benign prostate hyperplasia(BPH) and prostatic central gland cancer(PCG-Co) on diffusion weighted imaging(DW1), and to evaluate the value of DWI in differential diagnosis between two diseases. Methods 28 BPH and 26 PCGCa were scanned with conventional MR imaging and DWI, All the BPH and PCGCa were proved by patholo- gy. ADC values of PCGCa and BPH tissues were measured and analysed statistically. Results 26 BPH showed inhomogeneous and slightly high signal intensity in DWI, 2 BPH showed obviously high signal intensity and the mean value of ADC was (1.13±0.18)×10^-3mm^2/s. Among 26 PCGCa cases, 24 tumors showed obviously high signal intensity, and 2 tumors demonstrated slight high signal intensity. The mean ADC value of tumors was (0.89±0.18)×10^-3mm^2/s. There was significant difference in ADC value between two groups. Conclusion DWI has important value in differential diagnosis of BPH and PCGCa.
