摘 要:目的探讨外周血肿瘤抑素(Tumstatin)、血管内皮细胞生长因子(VEGF)检测在非小细胞肺癌进展中的临床应用价值。方法使用酶联免疫吸附实验(ELISA)检测24例为非小细胞肺癌患者及24例健康体检者的血小板与血清Tumstatin、VEGF的水平。运用免疫组化法检测24例肺癌石蜡切片中微血管密度(MVD)的表达。结果血小板、血清Tumstatin水平在肺癌恶性进展中降低,血小板、血清VEGF水平以及肺癌组织中MVD在肺癌恶性进展中升高;血小板、血清Tumstatin在肺癌淋巴结转移组与肺癌病灶有坏死组中的水平分别低于肺癌无淋巴结转移组、肺癌病灶无坏死组(P<0.05),而血小板、血清VEGF在肺癌淋巴结转移组与肺癌病灶有坏死组中的水平分别高于肺癌无淋巴结转移组与肺癌病灶无坏死组(P<0.05);MVD计数在肺癌有淋巴结转移组中的水平高于肺癌无淋巴结转移组(P<0.05),但在肺癌病灶有坏死组与肺癌病灶无坏死组间差异无统计学意义。结论外周血Tumstatin与VEGF之间的平衡关系对肺癌的血管生成可能起重要的调节作用;外周血血管形成调节因子的水平与肿瘤组织MVD密切相关,对非小细胞肺癌的病情进展起到很好的监测作用。Objective To investigate the clinical significance of platelet, serum vascular endothelial growth factor ( VEGF ) and Tumstatin determination in the progression of non - small cell lung cancer ( NSCLC ) . Methods The levels of VEGF and Tumstatin in platelet and sera of 24 patients with NSCLC and 24 healthy controls were determined by ELISA method. Microvessel density (MVD) in tumor tissues from 24 NSCLC patients were examined by immunohistochemistry. Results The levels of platelet and serum Tumstatin were decreased in lung cancer patients with increasing grades of malignancy, but the levels of platelet and serum VEGF and MVD count in cancer tissues were increased; the platelet and serum Tumstatin levels were lower in patients with nodal metastasis and necrosis than those in patients without nodal metastasis and necrosis ( P 〈 0. 05 ), respectively ; the levels of platelet and serum VEGF were higher in patients with nodal metastasis and necrosis than those in patients without nodal metastasis and necrosis (P 〈 O. 05 ) , respectively. Additionally, MVD count in patients with nodal metastasis was higher than that in patients without nodal metastasis. There was no statistically significant difference in the MVD count between patients with and without necrosis. Conclusion The balance between Tumstatin and VEGF in peripheral blood of lung cancer may play an important role in moducation of angiogenesis. The level of angiogenesis regulators in peripheral blood is closely correlated with MVD, which can play an critical role in monitoring NSCLC progress.
