CBCT应用于上颌第一磨牙近中颊根根管形态的研究

摘　　要：目的:利用锥形束CT(cone-beam computed tomography,CBCT)扫描上颌第一磨牙近中颊根,并分析其根管解剖形态,为将CBCT的成像优势用于临床病例的根管系统分类提供参考。方法:选取我院口腔科就诊的153名患者262个上颌第一磨牙,以40岁为界分为A(≤40岁)、B(>40岁)两组进行CBCT扫描,分别对两组上颌第一磨牙近中颊根根管系统的解剖结构进行观察分析。结果:被检牙的3根率为99.2%(260/262)。A组MB2发现率为80.4%,B组为67.2%(P<0.05)。A、B组最常见的根管类型均为VertucciⅣ型(2-2),分别为46.1%和37.5%,其次为Ⅱ型(2-1),分别为34.8%和32.5%。近颊根髓室底根管开口至分支处距离,A组为(2.2±1.2)mm,B组为(2.0±0.6)mm(P>0.05)。结论:上颌第一磨牙大多具有3根和VertucciⅣ型根管,MB2根管在40岁以上人群中发生率较高。AIM： To investigate the morphology of mesiobuccal root and canal of maxillary first molars by cone-beam computed tomography （CBCT）. METHODS： 262 maxillary first molars in 153 Chinese patients were scanned by CBCT. The patients were divided into group A （≤40 years ） and group B （ 〉 40 years ）. The MB2 canal incidence, the anatomical structure and the correlations between the incidence of MB2 canal and age were analyzed by the CT images. RESULTS： 260 of the 262 molars （99.2%）were with 3 roots, only 2 （0.8%）with 2 roots （merged buccal root） in one patient. The incidence of MB2 canal was 80.4% and 67.2% in group A and B respectively（P 〈 0.05 ）. The most common canal type of both groups was Vertucci type 1V （2 -2） ,46.1% in group A and 37.5% in group B; followed by type I] （2 -1 ）, 34.8% in group A and 32.5% in group B. The average distance between root canal orifice of the mesiobuccal root to the canal diverge into two canals was （ 2.2 ± 1.2 ） mm in group A and （2.0 ± 0.6） mm in group B （P 〉 0.05 ）. CONCLUSION ： Most maxillary first molars have 3 roots and with Vertucci 1V type canals. MB2 canal incidence in the individuals younger than 40 years is higher than in those older than 40.
