斑点追踪成像评价左心室舒张功能障碍患者心室扭转运动的研究

摘　　要：目的应用斑点追踪成像评价左心室舒张功能障碍患者左心室扭转运动的变化,并探讨其临床意义。方法选取左心室舒张功能障碍患者84例,按照左心室舒张功能障碍诊断标准分为3组,其中松弛性减低组28例,假性正常化组30例,限制性充盈组26例,另选取健康志愿者32例为对照组。应用斑点追踪成像在左心室短轴心底水平和心尖水平测量心底部峰值旋转角度及其达峰时间,心尖部峰值旋转角度及其达峰时间,计算左心室峰值扭转角度。结果松弛性减低组左心室峰值扭转角度较其他3组增大(P<0.05),而限制性充盈组左心室峰值扭转角度较其他3组减小(P<0.05)。结论松弛性减低患者左心室扭转运动明显增强,限制性充盈患者左心室扭转运动减小,随着左心室舒张功能的减低,左心室扭转运动逐渐减低。斑点追踪成像为心功能障碍患者机械力学的评价提供了一个新途径。Objective To evaluate left ventricular torsion in patients with diastolic dysfunction by speckle tracking imaging, and to investigate its clinical significance. Methods Eighty-four patients with diastolic dysfunction, including 28 cases of abnormal relaxation pattern, 30 cases of pseudonormal filling pattern and 26 cases of restrictive filling pattern were enrolled, and 32 healthy volunteers were in control group. Peak basal rotation, peak apical rotation at basal and apical short-axis views were measured with speckle tracking imaging,and peak left ventricular torsion was calculated. Results Peak left ventricular torsion was significantly increased in abnormal relaxation pattern group than that in other groups, and it was decreased in restrictive filling group compared with other groups （P 〈 0.05）. Conclusion Systolic torsion is significantly increased in patients with abnormal relaxation pattern, and declined in patients with restrictive filling pattern. Systolic torsion is gradually declined with the decrease of left ventricular diastolic function. Speckle tracking imaging may provide a new approach for evaluation of the mechanics of patients with cardiac dysfunction.
