卵巢早衰患者卵巢纤维化的超声诊断价值

摘　　要：目的探讨经阴道彩色多普勒超声对卵巢早衰患者卵巢纤维化的诊断价值。方法卵巢早衰组患者40例,月经周期正常的女性42例为对照组;经阴道彩色多普勒超声检查受检者卵巢体积变化、纤维条索状改变等卵巢纤维化的直接征象,并观察卵巢血流动力学参数、子宫体积和内膜的声像变化等卵巢纤维化的间接征象。结果卵巢早衰组卵巢体积不同程度萎缩,呈纤维化的直接征象,卵巢体积小于对照组(P<0.01);与对照组比较,卵巢早衰患者子宫内膜变薄(P<0.01),子宫体积变化不明显(P>0.05);卵巢间质动脉的收缩期峰值流速及舒张末期流速降低,阻力指数及搏动指数增高(P<0.01);卵巢血流信号稀少。结论超声检查是诊断卵巢早衰患者卵巢纤维化的重要手段,有助于评估卵巢早衰患者的卵巢功能。Objective To observe the diagnostic value of transvaginal color Doppler ultrasound in ovarian fibrosis of patients with premature ovarian failure. Methods Forty patients with premature ovarian failure （POF group）, and 42 women of childbearing age as control group underwent transvaginal color Doppler ultrasound examination. The direct signs of ovarian fibrosis in two groups, such as ovarian volume, fiber cord liked changes, and echo of ovary and the indirect signs such as the ovarian hemodynamie parameters, uterine volume and endometrium were observed and compared. Results Ultrasound observation showed the direct signs of fibrosis in the POF group, such as the ovarian atrophy in varying degrees, ovarian volume was significantly smaller than that of the control group （P 〈 0. 01 ）. Ovarian blood flow signal of POF was rare. Compared with the control group the endometrium become thinner （P 〈 0. 01 ）, the uterine volume did not change significantly （P 〉 0.05 ）, and there was significant difference in observed indexes including systolic peak velocity,end diastolic velocity,resistance index and pulsatility index between the two groups （ P 〈 0. 01 ） . Conclusion Color Doppler ultrasound is an important means for diagnosis of ovarian fibrosis, it is helpful to assess ovarian function of patients with premature ovarian failure.
