异丙酚芬太尼复合麻醉在美容整形手术中镇痛效果的观察

摘　　要：目的:观察异丙酚芬太尼复合麻醉在美容整形手术中的镇痛效果。方法:选取ASAI-Ⅱ级美容整形手术者66例,随机分为局麻组和复合组各33例。局麻组使用0.5%利多卡因局部浸润进行麻醉,复合组于消毒铺单后,给予芬太尼1μg/kg,2min后术区进行局麻药注射,切口给予异丙酚1mg/kg静注后以微量泵30～60μg/(kg.min)的速度静脉持续泵入;手术期间对两组镇痛镇静效果、心率和平均动脉压变化情况进行比较。结果:复合组镇静、镇痛等级为I级居多,局麻组Ⅰ级、Ⅱ级、Ⅲ级居多;复合组心率和平均动脉压变化情况均显著优于局麻组,差异具有统计学意义(P<0.05)。结果表明美容整形手术中应用异丙酚芬太尼复合麻醉可获得确切的镇痛镇静效果,手术者血压、心率等变化均趋于稳定。结论:异丙酚芬太尼复合麻醉的应用在美容整形手术中的镇痛效果十分突出,手术者可安静度过手术期,使手术舒适性及安全性得以有效提升,有利于医疗服务质量的全面提升,且整形美容手术患者对其镇痛效果十分满意,值得临床推广及应用。Objective To observe the propofol fentanyl compound anesthesia in cosmetic plastic surgery of analgesic effect. Methods Select ASAI - level II cosmetic plastic surgery is 66 cases were randomly divided into local anesthesia group and the composite group of each 33 cases. Local anesthesia group USES 0.5% lidocaine local infiltration anesthesia, compound group in disinfection draping, give fentanyl 1μg/kg,2 min after operation area for local anaesthetic injection, incision give propofol 1 mg/kg static note in micro pump 30-60μg/（kg-min）speed vein continuous pump into; During the operation of two groups of analgesic calm effect, heart rate and mean arterial pressure changes in comparison. Results The composite group of sedative and analgesic level for class I mostly, local anesthesia group in class I and class II, Ill strategy, Composite group of heart rate and mean arterial pressure changes were significantly superior to local anesthesia group, a statistically significant difference （P〈0.05）.The results show that cosmetic surgery application propofol fentanyl compound anesthesia may obtain the exact analgesic calm effect, the operation such as blood pressure and heart rate changes are stable. Conclusion Propofol fentanyl compound anesthesia used in cosmetic plastic surgery of analgesic effect is very outstanding, surgery can be quiet spend operation period, make the operation comfort and safety to enhance the medical service quality, which is beneficial to the overall ascension, and plastic cosmetic surgery patients to its analgesic effect is very satisfied,it is worth clinical popularization and application.
