摘  要:目的探讨磁共振弥散加权成像(DWI)在肺良恶性病变鉴别诊断中的应用价值。方法对33例肺病变行屏气弥散加权成像,对比分析良恶性病变在DWI像上信号强度(SI)及其表观弥散系数(ADC)是否存在显著差别。结果 19例恶性病变和14例良性病变在DWI像上的SI存在显著差异,如果以DWI中SI(病灶)/SI(胸壁)比值大于1.2作为判定恶性病变的标准,则本组病例的灵敏性为79%,特异性为86%,阳性预测值为88%,阴性预测值为75%,诊断准确率为82%。但良恶性病变的ADC值无显著性差异。结论磁共振弥散加权成像在肺病变良恶性鉴别诊断中具有一定的参考价值,可作为形态学鉴别的重要补充。Objective To investigate the application value of diffusion-weighted MRI in differentiating malignant from benign lung lesions. Methods 33 cases of lung lesions underwent the breath-hold Diffusion-weighted MRI (DWI), the differences in signal intensity (SI) and the value of apparent diffusion coefficient (ADC) were compared. Results The SI on DWI was significantly different between the 19 malignant and the 14 benign lung lesions. Taking 1.2 as a threshold, the ratio of SI(sllesion) to SI(slmuscle) had a sensitivity, specificity, positive predictive value, negative predictive value and accuracy of 79 %, 86 %, 88 ~, 75 % and 82 % correspondingly in the differentiation. The ADC values were not signifi- cantly different between the malignant and benign lung lesions. Conclusion Diffusion-weighted MRI may play an impor- tant role in differentiating malignant from benign lung lesions in addition to morphological judgement.
