妥洛特罗贴在小儿毛细支气管炎中的应用

摘　　要：目的比较妥洛特罗贴与雾化吸入沙丁胺醇在小儿毛细支气管炎中的作用,同时观察药物的不良反应。方法 60例毛细支气管炎患儿随机分为观察组和对照组各30例,观察组采用妥洛特罗贴片联合雾化吸入异丙托溴铵、布地奈德治疗;对照组采用雾化吸入沙丁胺醇、异丙托溴铵及布地奈德治疗,比较治疗前后2组患儿咳嗽、哮鸣音、排痰等症状评分及对日常生活和睡眠的影响评分,比较用药第1天和第7天的潮气肺功能,同时检测血、尿常规并观察是否有其他严重不良反应出现。结果 2组总有效率、咳嗽程度、哮鸣音、排痰情况及对日常生活的影响差异均无统计学意义(P>0.05),但睡眠质量差异有统计学意义(Z=1.974,P<0.05);2组用药第7天的潮气量、吸呼比、达峰时间比和达峰容积比均优于第1天(P<0.05),2组治疗第1天上述各指标差异均无统计学意义(P>0.05),但观察组第7天的达峰时间比和达峰容积比优于对照组(P<0.05)。结论妥洛特罗贴联合雾化吸入异丙托溴铵、布地奈德治疗毛细支气管炎疗效显著,不良反应少,使用方便,依从性好。Objective To compare the effect of tulobuterol patches and nebulized salbutamol on bronchiolitis in infants, and observe the adverse reaction. Methods Sixty infants with bronchiohtis were randomly divided into observation group and control group. There were 30 cases in each group. The observation group was given tulobuterol patches combined with nebulization ipratropium bromide and budesonide, whereas the control group was given inhalation of salbutamol, ipratropium bromide and budesonide. Symptom scores including coughing and wheezing, the effect scores of daytime lives and nocturnal sleep and the incidence of adverse reactions were recorded before and after treatment between two groups. The puhno- nary function tests were detected on the first day and the seventh day of medication. The routine blood test, routine urine test and serious adverse reactions were observed also in two groups. Results There were no significant differences in the total therapeutic effect, the level of cough, expectoration,, wheezing and the score of daytime lives between two groups （P 〉 0.05）, but nocturnal sleep was improved significantly in observation group （Z=1.974, P〈 0.05）. The tidal volume, TI/TE, TPTEF/TE and VPEF/VE were significantly improved after 7-d treatment in both groups （P 〈 0.05）, but no significant differences were found on the first day of treatment between two groups （P 〉 0.05）. The values of TPTEF/TE and VPEF/VE were significantly better in observation group than those in control group after 7-d treatment （P 〈 0.05）. Conclusion Tulobuteml patches combined with nebulized ipratropium bromide and budesonide have a certain effect in treatment of broncbiohtis in infants, showing faster effect, fewer adverse reactions and good compliance.
