摘 要:目的:观察有创与无创正压通气对呼吸衰竭患者的血浆脑钠肽(BNP)含量的改变及影响。方法:将60例呼吸衰竭患者随机分为观察组与对照组,每组30例,观察组使用有创正压通气治疗,并根据情况进行调节,对照组使用无创正压通气治疗,观察两组患者治疗前及治疗后2 h血浆脑钠肽浓度及血气分析结果。结果:观察组患者机械通气后血浆BNP浓度明显低于对照组,调节参数后进一步下降,且心率、呼吸及血气分析指标均较对照组患者有明显变化,差异有统计学意义(P<0.05)。结论:有创正压通气可以有效降低呼吸衰竭患者血浆BNP浓度,明显减轻心脏负荷,效果优于无创通气。Objective To observe the effect of non-invasive positive pressure ventilation on respiratory failure in patients with plasma brain natriuretic peptide(BNP) content change and influence.Methods 60 patients with respiratory failure were randomly divided into observation group and control group,30 cases in each group,the observation group using invasive positive pressure ventilation in the treatment,and adjusted according to the situation,the control group using noninvasive positive pressure ventilation in the treatment,the two groups were observed before and after treatment with 2 h of plasma brain natriuretic peptide concentration and the results of blood gas analysis.Results The concentration of plasma BNP in patients with mechanical ventilation in the observation group were lower than that of control group,after adjusting parameters decreased further, and heart rate,respiration and blood gas analysis were compared with the control group,patients have obvious changes,the difference was statistically significant(P 〈 0.05).Conclusion Invasive positive pressure ventilation can effectively decrease the concentration of BNP in plasma of patients with respiratory failure,significantly reduce the load on the heart,is better than that of noninvasive ventilation.
