血必净联合磷酸肌酸钠对脓毒症心肌损伤的保护作用

摘　　要：目的探讨血必净联合磷酸肌酸钠对严重脓毒症心肌损伤的保护作用。方法将90例急性生理学与慢性健康状况评分Ⅱ(APACHEⅡ)评分≥15分的脓毒症伴心肌损伤患者随机分为对照组、血必净组和血必净联合磷酸激酶钠组(联合组)。对照组给予常规治疗,血必净组在对照组基础上加用血必净静脉滴注,联合组在血必净组基础上给予磷酸激酶钠静脉滴注,均连续治疗7d。3组分别于治疗1、3、7d后检测血浆氨基末端B型钠尿肽前体(NT-proBNP)水平。结果 3组患者治疗1d后血浆NT-proBNP比较,差异无统计学意义(P>0.05)。治疗3、7d后血必净组、联合组血浆NT-proBNP较对照组下降,差异有统计学意义(P<0.01);且联合组优于血必净组,差异有统计学意义(P<0.05)。结论血必净对抗内毒素、抑制细胞因子和抗氧化作用能够减轻心肌损伤程度,联合应用磷酸激酶钠直接对损伤的心肌细胞供能取得的效果更为明显。Objective To investigate the protective effect of Xuebijing associated with Creatine phosphate sodium on myocardial damage of patients with severe sepsis.Methods Ninety patients with sepsis and acute physiology and chronic health evaluation Ⅱ（APACHEⅡ） score≥15,accompanied with myocardial damage,were randomly divided into a control group,Xuebijing group and Xuebijing combined with Creatine phosphate sodium group（combined group）.The control group was given conventional treatment,Xuebijing group plus Xuebijing intravenously,combined group were given Creatine phosphate sodium intravenously in Xuebijing group basis,treated continuously for 7d.3 groups were precursor the plasma N-terminal pro B type natriuretic peptide（NT-proBNP） level after treatment 1,3,7d.Results 3 groups of patients after 1d,plasma NT-proBNP comparied,there was no significant difference（P〈0.05）;After treatment 3d,7d,Xuebijing group,the levels of plasma NT-proBNP in combined group compared with the control group decreased significantly,the difference was statistically significant（P〈0.01）;and the combination group was better than that of Xuebijing group,the difference was statistically significant（P〈0.05）.Conclusion Xuebijing against endotoxin,inhibitor and antioxidant effect of cell can reduce the degree of myocardial injury,combined application of Creatine phosphate sodium directly to the injury of the myocardial cell energy supply has more significant effect.
