改良手法复位树脂绷带外固定治疗Colles骨折75例临床观察

摘　　要：目的观察改良手法复位树脂绷带外固定治疗Colles骨折的临床疗效。方法将150例Colles骨折患者随机分为2组,治疗组75例予改良手法复位树脂绷带外固定治疗,对照组75例予传统手法复位前臂夹板外固定治疗。观察2组疗效,比较治疗前后疼痛评分、肿胀评分变化,观察复位固定后再次移位情况。结果治疗组优良率90.28%,对照组优良率86.30%,2组优良率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。单纯桡骨远端骨折、桡骨远端合并尺骨茎突撒脱骨折总有效率均高于粉碎性骨折(P<0.05)。2组治疗后疼痛评分、肿胀评分均较本组治疗前降低(P<0.01),且治疗组降低更明显(P<0.05)。2组复位固定后再移位发生率比较差异无统计学意义(P>0.05)。结论改良手法复位树脂绷带外固定治疗Colles骨折较传统手法复位前臂夹板外固定疗效好。Objective To observe the effect of external fixation by resin bandage after improved manual re- duction on Colles fracture. Methods 150 patients with Colles fracture were randomly divided into two groups. 75 ca- ses in control group were treated by external fixation of forearm splint from traditional technique reduction. 75 cases in treatment group received external fixation by resin bandage after improved manual reduction. The effect, pain score, swells score, reduction and fixation shift again were observed. Results Excellent rate in treatment group （90.28%） was superior to that in control group （86.30%, P 〈 0.05）. The total effective rate of simple distal radius fractures, distal radius ulna styloid scattered off fractures was higher than that of comminuted fracture （ P 〈 0.05 ）. The pain score and swell score after treatment was decreased as compared with that before in two groups （P 〈0. O1 ）. The de- crease of treatment group was obvious （P 〈 O. 05）. There was no difference between two groups on the recurrent rate of reduction and fixation shift again （ P 〉 0.05 ）. Conclusion External fixation of Colles fracture by resin bandage after improved manual reduction is superior to that in External fixation of Colles fracture by Forearm splint after tradi- tional technique reduction.
