摘  要:目的探讨穿支供血的腓肠外侧皮神经营养血管皮瓣游离修复手掌背及其他较大部位皮肤软组织缺损的临床应用效果。方法选取严重手掌或者手背较大面积的软组织缺损创面患者16例,创面清创后以小腿中段腓动脉穿支为蒂、腓肠外侧皮神经营养血管链为轴心的皮瓣,游离移植修复软组织缺损创面,术后观察皮瓣成活情况,总结穿支血管的切取、皮瓣与受区血管的吻合搭配等技巧。结果临床治疗16例患者,皮瓣面积最大14cm×8cm,均以2支穿支为供血血管,皮瓣全部成活。结论腓肠外侧皮神经营养血管皮瓣轴心血管链及供血穿支解剖恒定,皮瓣质地薄软,血运丰富,供受区均不损伤主干血管,可作为修复较大面积的手掌背及其他部位软组织缺损创面的理想选择。Objective To investigate the clinical effect of perforators lateral sural neurocutaneous vascular flap free repair palm dorsal and other large area skin and soft tissue defect. Methods Regarded perforator pedicled, lateral sural cutaneous nerve nutrient vessel chain as the axis of the flap, and free repaired palm dorsal and other large parts of the defect of soft tissue. Observed flap cut scope, summarized the incision of the perforator flap and recipient vessel anastomosis collocation skills. Results The largest skin flap area of 16 cases was 14 cm ×8 cm, operated 2 branches as blood vessel, all flaps survived. Conclusion The lateral sural neurocutaneous vascular flap axis vessel chain and blood wear division section constantly. The flap is thin and soft with rich blood supply, can be used to repair large area palm back and other parts of the ideal soft tissue defects.
