2株山羊葡萄球菌药敏及诱导性耐药的检测

摘　　要：目的探讨患者山羊葡萄球菌药敏及诱导性耐药。方法用美国BD公司PhoenixTM100全自动细菌鉴定/药敏仪及其NMIC/ID-55鉴定/药敏板对患者血分离的2株山羊葡萄球菌(菌株92738、菌株100710)进行鉴定和21种抗菌药物敏感性检测,应用NMIC/ID-55鉴定/药敏板头孢硝噻吩试验检测其β-内酰胺酶,苯唑西林-头孢硝噻吩纸片法检测诱导性β-内酰胺酶,D试验检测大环内酯类和氨基糖诱导性耐药,苯唑西林纸片抑菌环周菌落乳胶凝集法检测诱导性青霉素结合蛋白2a。结果 PhoenixTM100细菌鉴定仪鉴定该二菌均为山羊葡萄球菌,可信度分别为95%、99%;1株全敏感,NT试验阴性。1株仅对青霉素、四环素耐药,NT试验阳性;该二菌D试验、纸片抑菌环周菌落青霉素结合蛋白2a乳胶凝集法均诱导阴性。结论这2株山羊葡萄球菌临床分离株耐药性较低,其机制为产β-内酰胺酶,未检出诱导性β-内酰胺酶、诱导性青霉素结合蛋白2a、大环内酯类和氨基糖耐药。Objective To investigate the susceptibility and inducible resistance for staphylococcus caprea. Methods Two strains of staphylococcus caprea was separated and 21 kinds of sensitivity of antimicrobial drugs were tested by BD＇s phoenixTM 100 automatic bacteria identification/susceptibility meter （strain 92738, strain 100710）, β-lactamase was detected by NMIC/ID-55 identification/susceptibility meter nitrocefin test （NT）, induced β-lactamase was detected by benzene azole nonsevere cephalosporins nitrate thiophene paper method, induced resistance of Macrolides and amino sugars was detected by D-tests. Inducible PBP2＇was detected by benzene azole westwood disc bacteriostatic an nulus colonies latex agglutination tests. Results Two strains of staphylococcus caprae were identified by the phoenixTM 100 bacteria identification instrument, the confidence level of it were 95% and 97% respectively; one strain was susceptibility to all drugs, NT test was negative; the another were resistant to penicillin and tetracyclin, NT test was position; D -tests and slip bacteriostatic annulus colony penicillin binding protein 2 a latex agglutination method of the two strains were negative. Conclusion The susceptibility of 2 strains of staphylococcus caprae from blood sample of patients were low, the mechanisms is to produce β-lactamase, without induced β-lactamase, induced penicillin binding protein 2a, macrolides and amino sugars.
