摘  要:目的对比分析等离子双极与单极宫腔镜子宫内膜电切术中组织损伤,为临床应用提供有效的参考依据。方法在体实验:随机抽取需行子宫内膜电切术的功能失调性子宫出血(功血)或多发性子宫内膜息肉患者30例,随机分为双极组和单极组;离体实验:随机抽取因多发性子宫肌瘤、功能失调性子宫出血等行子宫切除术的离体子宫标本(排除子宫内膜恶性病变)30例,随机分为双极组和单极组,每组各15例,对NADH-d组织化学染色后的单极组与等离子双极组电极作用下方的组织热损伤深度进行比较。结果无论是在体实验或是离体实验,单极组的组织热损伤带均较等离子双极组的组织热损伤带深(t在体=2.39,t离体=7.49,P<0.05)。结论等离子双极宫腔镜电切术对周围组织的损伤较单极系统轻,结合等离子双极电切不需要使用负极板,电流不通过人体,安全性好,用生理盐水做膨宫介质,能有效的防止TURS的发生等优点,证实等离子双极宫腔镜可广泛地应用于临床中。Objective To compare and analyze the clinical application of plasma bipolar and monopolar hysteroscopic endometrial transurethral surgery, tissue damage, and provide effective reference for. Methods In vivo experiments: random sample required line transurethral resection of endometrial dysfunctional uterine bleeding (DUB) or multiple endometrial polyps in 30 patients were randomLy divided into bipolar group and unipolar group; vitro experiment: ran- domLy selected 30 cases of multiple uterine fibroids, dysfunctional uterine bleeding, hysterectomy and other isolated uterine specimens (exclude endometrial malignant lesions) were randomLy divided into bipolar group and unipolar groups compares with 15 cases in each tissue thermal damage depth of the unipolar group NADH-d staining plasma bipolar group electrode role below. Results Either in vivo or in vitro experiments, the unipolar group compared with the tissue thermal damage with plasma bipolar group with deep tissue thermal injury (tt~ = 2.39, t^o = 7.49, P 〈 0.05 ). Conclusion Plasma bipolar hysteroscopic electric resection compared with unipolar system light damage to the sur- rounding tissue, combined plasma bipolar transure/hral do not need to use the negative plate, not through the current human security, with saline distention media , can effectively prevent TURS occurrence advantages confirmed plasma bipolar hysteroscopy can be widely used in clinical.
