个体化护理在内镜下胆汁引流术中的应用

摘　　要：目的提高患者内镜下胆汁引流术后护理效果,推动个体化护理模式推广程度。方法对2009年9月～2012年4月在我院行胆汁引流术的88例患者临床治疗资料进行回顾性分析,根据资料中护理方式不同将患者分为个体化护理组与对照组两组,每组44例,总结对比两组护理结果。结果 88例患者置管成功86例(成功率97.7%),2例患者因肝门部狭窄严重,导丝未能顺利通过而置管失败。两组治疗效果比较无明显差异,P>0.05;治疗总有效率:个体化护理组97.3%,对照组86.4%,P<0.01。患者术后均未出现明显并发症。个体化护理组护理满意度93.2%,对照组护理满意度29.5%,P<0.01。结论内镜辅助下行胆汁引流术操作简单易行,无明显并发症,且能取得良好治疗效果;对胆汁引流术患者行个体化护理能有效缓解患者围手术期焦虑程度与提高患者配合程度,提高最终护理成效。Objective To observe the endoscope assisted biliary drainage effect and individualized nursing effect of postoperative patients. Methods Clinical data of 88 patients receiving biliary drainage in this hospital were retrospec- tively analyzed from September 2009 to April 2012, reushs and total effective rates of treatments for patients were compared; They were divided into individual nursing group and control group by different nursing way （n = 44, results of contrast nursing）. Results There were 86 cases of 88 patients showed successful catheter indewelling, the successful rate was 97.7%, the guide wires of two cases of patients didn＇t pass due to serious epatic portal vein stenosis. There was no obvious difference in treatment effects of two groups, P 〉 0.05 ;There were significant differences in the total effective rate, P 〈 0.01. No obvious complications were found in postoperative patients. The nursing satisfaction of individualized nursing group was 93.2%, and that of the control group was 29.5%, there was significant difference in data contrast of two groups, P 〈 0.01. Conclusion The operation of endoscopic auxiliary descending biliary drainage is simple, no significant complication is found, and good treatment effect can be achieved. It could effectively relieve perioperative anxiety level of patients with individualized nursing of biliary drainage and improve the coordiantaive degree of patients, enhancing final nursing effect.
