摘  要:目的探讨16层螺旋CT在马蹄肾血供和肿瘤中的诊断价值。方法对3例马蹄肾患者先行16层CT平扫,然后行肾皮质期、实质期及排泄期增强扫描,将扫描数据经薄层重建后传至工作站,采用多平面重组(MPR)、容积再现(VR)及最大密度投影(MIP)技术进行后处理,观察马蹄肾融合的部位及其组织结构以及血供和合并肿瘤情况。结果马蹄肾并发肾母细胞瘤1例,马蹄肾并发卵黄囊瘤1例,马蹄肾并发肝脏错构瘤1例。结论 16层螺旋CT能够清楚显示马蹄肾的整体形态和细微解剖结构,尤其是血供和合并肿瘤情况,为临床外科治疗提供解剖学依据。Objective To study the diagnosis value of 16-slice spiral CT for renal cell carcinoma and upper ufina~ tract transitional cell arcinoma in horseshoe kidney. Methods Three patients with horseshoe kidney underwent 16- slice spiral CT plain scan, then kidney cortical phase, parenchymal phase and excretory phase contrast-enhanced CT scan, and all data sets reconstructed by thin slices were transferred to the workstation, and post-processing techniques including MPR,VR and MIP were performed for evaluating the fusion site and tissue, blood supply and tumor. Results In horseshoe kidney, the renal lower poles of the right and left kidneys were fused together by an isthmus of function- ing renal parenchyma in 3 cases.2 case underwent surgical treatment, pathological diagnosis were yolk sac tumor and Wilms" tumor, 1 case were loss to follow-up, CT diagnosis of liver nodular hyperplasia or hamartoma. Conclusion 16- slice spiral CT techniques can clearly demonstrate the overall shape and anatomic structure in detail, especially for the blood supply and tumor of horseshoe kidney.
