妊娠中期糖代谢异常孕妇胰岛自身抗体的分布及其对临床特征的影响

摘　　要：目的分析妊娠中期糖代谢异常孕妇胰岛自身抗体分布及其对临床特征的影响。方法选取妊娠中期糖代谢异常孕妇82例,检测患者谷氨酸脱羧酶自身抗体(GADA),胰岛细胞自身抗体(ICA)及胰岛素自身抗体(IAA)。依据检测结果将所有患者分为阳性亚组和阴性亚组,比较2亚组年龄、腰围、妊娠史、糖尿病家族史和总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)。结果妊娠期糖尿病组和妊娠糖耐量受损组患者GADA、ICA、IAA阳性率及总阳性率比较差异无统计学意义(P>0.05),IAA阳性率均明显高于各组内GADA及ICA阳性率,差异均有统计学意义(P<0.05)。2亚组年龄、腰围、妊娠史、糖尿病家族史和TC、TG、HDL-C、LDL-C比较差异无统计学意义(P>0.05)。结论妊娠期糖代谢异常患者胰岛自身抗体发生率较高,其中胰岛素自身抗体的阳性率最高,且妊娠糖尿病与妊娠糖耐量受损者胰岛自身抗体阳性患者临床特征不明显。Objective To analysis the distribution and the affect on clinical characteristics of second trimester of pregnancy women with abnormal glucose metabolism.Methods Selected 82 cases second trimester of pregnancy women with abnormal glucose metabolism.Dtected the glutamic acid decarboxylase autoantibodies（GADA）,islet cell autoantibodies（ICA） and insulin autoantibodies（IAA） of 82cases patinets.According to the detection result,82 cases patients were divided into positive subgroup and negative subgroup.Compared the age,waist circumference,pregnancy history,family history of diabetes,and total cholesterol（TC）,triglyceride（TG）,high density lipoprotein cholesterol（HDL-C）,low-density lipoprotein cholesterol（LDL-C） of 2 groups.Results The difference of GADA、ICA、IAA positive rate among gestational diabetes mellitus,pregnancy impaired glucose tolerance and the total rate was no statistically significant（P 0.05）.The IAA total rate were higher than the positive rate of GADA and ICA among 3 groups（P 0.05）.The difference of age,waist circumference,pregnancy history,family history of diabetes and TC,TG,HDL-C,LDL-C was no statistically significant（P 0.05）.Conclusion The patients with abnormal glucose metabolism during pregnancy has a high incidence of islet autoantibody.IAA has the highest positive rate.The clinical features of gestational diabetes and gestational impaired glucose tolerance with islet autoantibody positive was not obvious.
