摘 要:目的观察早期持续d,N量他达拉非应用对海绵体神经(CN)损伤后勃起功能障碍(ED)大鼠阴茎海绵体纤维化的改善作用。方法将36只雄性SD大鼠随机分为治疗组、CN损伤组、假手术组,每组12只。治疗组、CN损伤组均予钳夹损伤CN。治疗组大鼠手术当天即开始给予小剂量他达拉非(2mg/kg,加入饮水中)灌胃治疗,CN损伤组不作任何治疗,假手术组仅对CN进行暴露,不作任何处理。术后4周行阿朴吗啡(APO)试验、阴茎转化生长因子-β1(TGF-β1)免疫组织化学分析、阴茎Masson染色分析海绵体平滑肌/胶原纤维比率。结果APO试验结果:术后4周进行,观察30min,治疗组大鼠阴茎勃起率和平均勃起次数分别为54.55%(6/11)和(1.12±0.81)次,CN损伤组均为0,假手术组为100%(12/12)和(2.11±0.86)次,治疗组阴茎勃起率及平均勃起次数均较CN损伤组显著增加(P〈0.05),显示术后4周治疗组较CN损伤组勃起功能好。TGF-β1免疫组织化学结果:治疗组、CN损伤组、假手术组大鼠阴茎海绵体TGF-β1相对表达强度分别为14.12±3.72、26.45±5.33、8.37±1.25,治疗组较CN损伤组大鼠阴茎海绵体TGF-β1表达显著增强(P〈0.05)。阴茎Masson染色结果:术后4周治疗组、CN损伤组、假手术组大鼠阴茎海绵体平滑肌/胶原纤维比例分别为0.31±0.08、0.20±0.05、0.52±0.11,治疗组较CN损伤组平滑且几/胶原纤维比例湿著升高(P〈0.05)。结论早期持续小剂量他达拉非早期治疗可改善CN损伤后阴茎海绵体纤维化,有助于阴茎勃起功能的恢复。Objective To investigate the preventive e'ffect of early chronic tadalafil administration on corporal fibrosis induced by cavernous nerve injury (CNI) in rats. Methods Thirty-six male Sprague- Dawley rats were randomly divided into three groups: no CNI (sham, n = 12), bilateral CNI exposed to ei- ther no tadalafil ( CN crush, n = 12) or tadalafil ( treatment, n = 12) at a dose (2 mg/kg) daily postoperation for 4 weeks. At the time point, we assessed erectile function by apomorphine test. Then immunohisto-chemical antibody staining was performed for transforming growth factor-β1 (TGF-β1). And the Masson staining was used to quantitatively analyze the ratio of smooth muscle to collagen fibers in corpus caverno- sum. Results Penile erection was observed in 54. 55 % (6/11) of the rats for ( 1.12±0. 81 ) times within 30 min in treatment group, as compared with 0% (0/11) of the rats for (0. 00±0. 00) time in CN crush group (P〈O. 05), and 100% (12/12) of the rats for (2. 11±0.86) time in sham group (P〈0.05). The expression of TGF-β1 was significantly weaker in treatment group than in CN crush group ( 14. 12±3.72 vs. 26.45±5.33, P 〈0. 05). The ratio of smooth muscle to collagen fibers in corpus cavernosum in treatment group was significantly higer than that in CN crush group ( 0. 31±0. 08 vs. 0. 20 ±0. 05, P 〈 0. 05 ). Conclusion Early chronic low-dose tadalafil administration can improve fibrosis of corpus caverno- sum induced by CNI and contributes to restoration of erectile function.
