摘  要:目的:分析VSD技术对创伤后阔筋膜张肌下感染治疗的影响。方法:2组18例严重的大腿外侧软组织挫裂伤病人,伤后出现不同程度阔筋膜张肌下感染;其中第一组9例行常规清创、换药、引流、抗感染及对症支持治疗,第二组9例清创后采用VSD技术,同时予以抗感染及对症支持治疗,待感染控制后创面未愈合者行二期植皮治疗。对2组病人住院期间的清创次数、住院时间、住院费用进行对比分析。结果:所有患者感染控制、创面完全愈合后出院,随访9个月至3年,平均17.6个月,无创面溃破及感染复发。住院期间第一组(未采用VSD组)平均清创次数3.9次,平均住院时间57.2 d,平均住院费用15783.3元;第二组(VSD组)平均清创次数2.0次,平均住院时间47.3 d,平均住院费用10781.1元。结论:阔筋膜张肌下感染后彻底的清创并采用VSD治疗可以避免感染扩散,减少清创次数,缩短病程,提高疗效,减少住院费用。Objective:To reveal and comparative analyse the influence of vaccum sealing drainage (VSD) technology in the treatment of post- traumatic infection under the tensor fasciae latae. Methods:All 18 patients who suffered serious soft tissue contusion and laceration of the lateral thigh and with different degrees of infection under the tensor fasciae latae after injury were divided into 2 groups, 9 cases in each groups. Group one accepted conventional debridement, dressing change, drainage, anti - infective and supporting treatment ; group two accepted vaccum sealing drainage (VSD) technology except the upper treatments. When the infection was controlled, whose wound surface was not healed accepted skin grafting. The debridement times, hospital stays and hospitalization expenses during their hospitalization of the 2 groups were comparative analysed. Results: All of the patients discharged after the infective wounds healed well and were followed up for 9 months to 3 years, there was no wound diabrosis and no infection recurred. During the hospitalization, group one's average debridement times was 3.9 times, hospital stays was 57. 2 days, hospitalization expenses was 15783.3 yuan; group two's average debridement times was 2. 0 times, hospital stays was 47. 3 days, hospitalization expenses was 10781.1 yuan. Conclusions: If the infection occurred, through debridement and vaccum sealing drainage (VSD) technology using can avoid infection spreading, reduce debridement times, shorten the length of the disease, enhance the curative effect, and reduce hospital expenses.
