病毒性肝炎的病位证素

摘　　要：目的确立病毒性肝炎的常见病位证素。方法通过中医临床诊疗信息采集系统采集病毒性肝炎病例19 341份。应用Visual FoxPro 9.0编程统计证候诊断以及湿热证和肝胆湿热证对应的症状、方剂和中药。以病位证素为参考,从证候诊断中提取病位证素,对病位证素进行统计分析,并与行业标准进行对比。结果病毒性肝炎涉及的病位证素有13个,分别为肝、脾、胆、肾、胃、心、肺、经络、大肠、脑、表、肌肤和下焦,出现频数较高的依次为肝、脾、胆、肾、胃。结论肝胆湿热证不是传统的中医证候,而是在西医学引进中国后由近代医家提出的。肝胆湿热证应该称为脾胃湿热证。病毒性肝炎的主要病位证素是脾而不是肝。Objective To determine common positional syndrome factors of viral hepatitis. Methods TCM collection system of clinical diagnosis and treatment information was applied to select 19 341 cases of viral hepatitis. The syndrome diagnosis and symptoms, formulas and Chinese medicinal corresponding to dampness-heat syndrome and liver-gallbladder dampness-heat syndrome were counted by using Visual FoxPro9.0 software. Referring to the positional syndrome factors, positional syndrome factors were extracted from syndrome diagnosis, analyzed statistically and compared with industrial standard. Results There were 13 positional syndrome factors involving viral hepatitis, including liver, spleen, gallbladder, kidney, stomach, heart, lung, meridian, large intestine, brain, the exterior, skin and lower energizer. The positional syndrome factors with higher frequency were, in turn, liver, spleen, gallbladder, kidney and stomach. Conclusion Liver-gallbladder dampness-heat syndrome is not a traditional TCM syndrome, which is proposed by modern physicians after Western medicine was introduced into China. Liver-gallbladder dampness-heat syndrome should be called spleen-stomach dampness-heat syndrome, and the main positional syndrome factor of viral hepatitis is spleen rather than liver.
