摘 要:目的采用相似肌纤维型的骨骼肌移植治疗小儿前臂Volkmann挛缩。方法以肌球蛋白ATPase法研究 4具小儿尸体前臂 (双侧 )的肌纤维型。选用相似肌纤维型的骨骼肌移植治疗小儿前臂Volkmann挛缩 8例。结果垂腕改善 ,可背屈 45° ;爪形手消失 ,可伸屈 ;手指功能恢复。结论应用相似肌纤维型的骨骼肌移植治疗小儿前臂Volkmann挛缩 ,方法简单可行 。Objective To study the distribution of muscle fiber types of forearm and to explore the suitable skeletal muscle to transplant for treatment Volkmann's stricture of forearm in children.Methods The distribution of muscle fiber types of forearm were studied by using myosin ATPase reaction in autopsy.Samples were taken from 4 cadavers (8 side)within 24 hours after accidental death from 3 5years old.The similar distribution of muscle fiber types of skeletal muscles were selected to transplant for Volkmann's stricture in 8 children.Results The muscle fiber types of forearm may divided as typeⅠ and typeⅡ,the percanfage of distribution was as follows∶the flexor digitorum super ficialis was 55.0±6 7;the digitorum profundus was 55 2±5 9;the extensor digiturum was 53 4±3 7; the palmaris longus was 49 3±1 3;and the flexor carpi ulnaris was 48 3±1 3.The percentage of type 1 with the muscle fibers of forearm were similar ( P >0 05).The therapeutic outcome of Volkmann's stricture in 8 children were satisfactory.Conclusion The palmaris longus of healthy forearm was choice to transplant for Volkmann's construction of forearm in children.
