氨氯地平联合替米沙坦治疗原发性高血压的疗效及对血清脂联素的影响

摘　　要：【目的】探讨氨氯地平联合替米沙坦治疗原发性高血压的临床疗效及对血清脂联素（ADP）水平的影响。【方法170例原发性高血压患者随机分为氨氯地平组和替米沙坦组，氨氯地平组给予氨氯地平联合替米沙坦联合治疗，替米沙坦组给予替米沙坦单药治疗，连续治疗30d后评价疗效，并选择30例健康者为对照组，测定原发性高血压组和对照组血清ADP水平。【结果】原发性高血压组血清ADP为（9．57±3．19）ng／mL，对照组为（17．63±5．51）ng／mL，高血压组显著低于对照组（t=6．934，P〈0．05）。氨氯地平组临床疗效总有效率为94．3％（33／35），替米沙坦组为68．6％（24／35），氨氯地平组显著高于替米沙坦组（X2=6．405，P〈0．05）。治疗后两组血压明显改善，SBP、DBP水平下降，血清ADP水平上升，与治疗前比较差异均有显著性（P〈0．05）；而且氨氯地平组与替米沙坦组比较差异有显著性（P〈0．05）。【结论】氨氯地平联合替米沙坦能有效降低原发性高血压患者血压和升高血清ADP水平，是治疗原发性高血压的一种安全有效方法。[Objective] To explore the clinical efficacy of amlodipine combined with telmisartan for the treatment of essential hypertension（EP） and its impact on serum adiponectin（ADP）. [Methods]Totally 70 EP patients were randomly divided into amlodipine group and telmisartan group. Amlodipine group was given the combination treatment of amlodipine and telmisartan, while telmisartan group was given only telmisartan. After 30 days of continuous therapy, the clinical efficacy was evaluated. Other 30 healthy people were chosen as the control group. Serum ADP of EP patients and healthy controls were detected. [Results] Serum ADP level of EP group and control group was （9.57 ± 3.19）ng/mL and （17.63 ± 5.51）ng/mL, respectively. Serum ADP of EP group was significantly lower than that of control group（t = 6. 934, P 〈0.05）. The clinical total effective rate of amlodipine group and telmisartan group was 94.3 % （33/35） and 68.6 % （24/35）, respectively. The total effective rate of amlodipine group was significantly higher than that of telmisartan group（x2= 6. 405, P 〈0. 05）. After treatment, blood pressure of two groups improved significantly, and SBP and DBP decreased, and serum ADP level increased, and there was significant difference between before and after treatment（ P 〈0.05）. And there was significant difference between amlodipine group and telmisartan group（ P 〈0.05）. [Conclusion] Amlodipine combined with telmisartan can effectively improve blood pressure and elevate serum ADP. Therefore it is a safe and effective method for the treatment of EP.
