抗生素在急性上呼吸道感染患者中的应用合理性分析

摘　　要：目的观察复方甲氧那明联合苯丙哌林治疗上呼吸道感染后咳嗽的治疗效果,为临床上合理用药提供指导。方法将在本院接受治疗的164例上呼吸道感染后咳嗽的患者分为A、B、C 3组,A组54例采用复方甲氧那明、苯丙哌林治疗;B组55例采用甲氧那明、苯丙哌林和抗菌药物治疗;C组55例单独采用抗菌药物(头孢菌素类、大环内脂类、喹诺酮类药物)治疗,疗程为2周。根据3组患者咳嗽、咳痰的得分以及不良反应情况来判断药物疗效。结果治疗后A、B组患者咳嗽和咳痰症状得分均低于C组,比较后差异均具有统计学意义(P<0.05);3组患者在治疗过程中均出现了不同程度的不良反应,与C组相比,A、B两组患者头晕、嗜睡及便秘症状的发生率明显升高(P<0.05)。上述不良反应属轻微或短暂性,在停药后均消失。结论复方甲氧那明联合苯丙哌林对上呼吸道感染后的咳嗽症状有明显的改善作用,无细菌感染症者,不需要使用抗菌药物治疗,抗生素应在熟知病因、症状的情况下进行合理性使用。Objective To observe the curative effect of compound methoxyphenaminecombined with benproperiue on cough after acute upper respiratory infection （AURI）,and provide appropriate direction for clinical treatment. Methods 164 cases of URI patients who were admitted into our hospital were divided into A,B,C three groups at random.A group（54 cases） was treated with CM and BP; B group （55 cases） was treated with CM,BP and antibiotics;C group was treated with antibiotics only （CEPs,Macrolides,Quinolones）, the treatment course was two weeks.The curative effect was evaluated by the scores of cough, sputum and adverse reaction among the three groups. Results The scores of cough and sputum of A,B group were lower than that in C group（P 〈 O.05）,varying degrees of adverse reaction occurred in the three groups during the therapeutic process.Compare with C group,the occurrence rate of dizziness,cons tipation,drowsiness in A group and B group was obviously increased（P 〈 0.05）. However,these symptoms were transient and slight,and adverse events disappeared after drug withdrawal. Conclusion CM combined with BP can obviously improve the cough symptom of AURI, and no infection occurred,also no need antibiotic for treatment.Antibiotics should be rational use in the situation of being familiar with pathogenesis and symptoms.
