摘 要:目的降低极低出生体重儿PICC导管堵管率。方法应用RCA分析法,从人、机、物、法、环等因素,回溯性分析引起极低出生体重儿PICC导管堵管的各种近端原因,通过分析归纳确定根本原因,针对根本原因,在理清发生原因与结果关系的基础上,制定预防导管堵塞的整改措施和流程指引并落实。结果试验组通过RCA分析,并落实预防导管堵塞的整改措施和流程指引,PICC堵管发生率为7.14%,比较未行RCA分析前的对照组PICC44.44%堵管发生率明显下降,两组差异有统计学意义(P<0.05)。结论应用RCA分析法,有助降低极低出生体重儿PICC导管堵管率。Objective To reduce the PICC catheter plugging rate of very low birth weight infants. Methods From the aspects of human,machine,material,law,environment and other factors,the RCA analysis method was used to analyze the proximal causes of PICC catheter plugging of very low birth weight infants;The root causes were determined through analysis and conclusion;Relevant rectification measures and process guidance to prevent catheter plugging were developed and implemented on the basis of clearing the relationship between the root causes and results. Results The experimental group received rectification measures and process guidance to prevent the catheter plugging after RCA analysis and had a PICC catheter plugging rate of 7.14%,which significantly reduced compared to the 44% of the control group who did not receive RCA analysis,with statistically significant difference between the two groups (P 〈 0.05). Conclusion The application of RCA analysis method is conducive to reducing the PICC catheter plugging rate of very low birth weight infants.
