NT-proBNP联合TDI评价冠心病患者PCI术前后心功能的临床价值

摘　　要：目的探讨氨基末端脑利钠肽前体(NT-proBNP)联合组织多普勒成像(TDI)在冠心病患者经皮冠状动脉介入治疗(PCI)术前后心功能评估中的临床价值。方法 51例冠心病患者行PCI术治疗,应用ELASA方法测定血浆NT-proBNP浓度,同时应用常规超声心动图超声测量左室收缩末期前后径、舒张末期前后径、房室瓣口舒张早期、晚期血流峰值速度及其比值(E/A)、左室射血分数(LVEF);应用TDI技术观察房室瓣环运动,测量其舒张早期、舒张晚期及收缩期运动的峰值速度(Em、Am、Sm)。E/A、Em/Am作为评价心室舒张功能的指标,左室缩短分数(FS)、Sm及LVEF作为评价心室收缩功能的指标。对比患者PCI术前后各参数的改变。结果(1)51例冠心病患者行PCI术后2周,血浆NT-proBNP浓度水平下降,Em/Am上升的幅度比E/A明显,其余指标与术前比较,差异无统计学意义(P>0.05)。(2)术后3个月与术前比较,除E/A外,其他指标差异有统计学意义(P<0.05)。结论血浆NT-proBNP水平联合组织多普勒成像可用于冠心病患者PCI术前后心功能的评估,为患者的后续治疗提供诊疗依据。Objective Toexplorethe clinical value of NT-proBNP combined with TDI in evaluation of CHD patients＇Cardiac function before and after PCI operation. Methods The serum NT-proBNP levels of 51 cases of patients with CHD during PCI operation were detected by ELISA method.The values of LVESD,LVED,early diastolic atrioventricular valve orifice,end diastolic velocity of blood flow and their E/A rati0,LVEF were measured by conventional echoeardiography at the same time.To observe the atrioventricular （AV） plane motion by TDI technology,and measure the early,end diastolic and systolic peak velocity（Em,Am,Sm）.E/A,Em/Am could be considered as a parameter of ventricular diastolic function,and FS,S and LVEF were applied as index of ventricular systolic function. The change of parameters were compared before and after PCI operation. Results （1）The serum NT-proBNP levels decreased in 51 cases of patients with CHD after 2 weeks of PCI operation,Em/Am rose more rapidly than E/A,other indicators had no significant difference compared with pre-operation（P〉 0.05）.（2）There were significant differences of all the indicators except E/A before and after 3 months＇operation（P 〈 0.05）. Conclusion NT-proBNP combined with TDI can evaluate Cardiac function of CHD patients before and after PCI operation,provide reference for future clinical diagnosis and treatment of CHD patients.
