摘  要:目的观察重组人脑利钠肽(recombinant human brain natriuretic peptide,rhBNP)用于超高龄男性患者慢性心力衰竭急性加重时的疗效及安全性。方法 25例年龄大于80岁的超高龄慢性心力衰竭急性加重男性患者,在积极病因治疗及抗心力衰竭治疗基础上,给予rhBNP治疗,观察呼吸困难症状、心率、血压、呼吸频率、尿量、脑利钠肽、左心室射血分数及肝、肾功能的变化。结果 rhBNP治疗后呼吸困难症状明显好转12例、好转9例、无变化2例、死亡2例,显效率为48%,总有效率84%。rhBNP治疗后,心率、呼吸频率、脑利钠肽均有显著下降,尿量、左心室射血分数增加。主要不良反应为低血压,对肝功能及肾功能无明显影响。结论 rhBNP用于超高龄慢性心力衰竭急性加重患者有效、安全、耐受性好。ObjectiveTo observe the efficacy and safety of recombinant human brain natriuretic peptide (rhBNP) in elderly male patients with acute exacerbation of chronic heart failure. MethodsTwenty five male patients older than 80 years old with acute exacerbation of chronic heart failure were given rhBNP for treatment based on standard anti heart failure therapy. Dyspnea, heart rate, blood pressure, respiratory rate, urine output, BNP, left ventricular ejection fraction, liver and kidney function changes were observed. ResultsDyspnea was significantly improved in 12 cases, improved in 9 cases,no change in 2 cases and 2 cases death. The effective rate is 48%, the total efficiency is 84%. After using rhBNP, the heart rate, respiratory rate, BNP were significantly decreased, and the urine output, left ventricular ejection fraction were increased. The main adverse reaction of rhBNP is low blood pressure. There were no significant effect on liver function and renal function. ConclusionrhBNP used in elderly male patients with acute exacerbation of chronic heart failure is effective, safe and well tolerated.
