孤立性蝶窦病变78例临床诊治体会

摘　　要：目的探讨孤立性蝶窦病变的临床特点和治疗方法,提高诊断率和治愈率。方法回顾分析78例孤立性蝶窦病变患者的临床表现、影像学特点及鼻内窥镜手术治疗效果。结果 78例患者中,蝶窦炎32例,蝶窦真菌24例,蝶窦粘液囊肿18例,蝶窦息肉4例。临床表现为头痛者56例(71.8%)、回缩带涕及涕中带血者26例(33.3%)、视力模糊或下降、眼球运动障碍者14例(17.9%)。全部患者术前均行CT及鼻内窥镜检查,术后病理证实。结论孤立性蝶窦病变临床表现无特异性,CT及鼻内窥镜检查可提高本病诊断率,内窥镜鼻窦微创手术是治疗本病的首选。Objective To investigate the imaging of breast cancer in MRI dynamic enhanced scan and DWI, and the value in diagnosing the lesions＇ quality . Methods Collected and analyzed MRI imaging data of fifty cases breast cancer patients confirmed by pathology in our hospital. Results Breast caner presented slightly low signal by MRI plain csan and T1WI, class or slightly high signal by T2WI. The tumor＇s shape is irregular, some have leaflet, fuzzy boundaries, visible spicule sing, were uneven reinforcement by enhancement scan. The time-signal intensity curve is given priority to HI type curve, which take up 68% ; ]1 type curve is less, take up 22% ; I type curve at least, only 10%. Thy early intensive rate of invasive cribriform cancer was highest, infiltrating ductal carcinoma and papillary carcinoma in the next place, and mucous adenocarcinoma was lowest, compare the four cancer, the difference was statistically significant （P 〈 0. 05 ） ;Compare peak reinforcement rate, the difference didn＇t have statistical significance （ P 〉 0.05 ）. The ADC of the lesions is （0.98 ± 0.23 ） ×10-3mm2/s, the ADC of the offside normal breast is （2.09 ±0. 22） ×10-3mm2/s, the difference was statistically significant （P 〈0. 05）. Conclusion MRI dynamic enhanced scan and DWI for breast cancer＇s qualitative diagnosis has important clinical value, it is worth clinical application.
