乙型肝炎血清学标志物的不同模式与前S1抗原的相关性研究

摘　　要：目的探讨乙型肝炎血清学标志物的不同模式与前S1抗原(Pre-S1Ag)的关系。方法应用酶联免疫吸附法(ELISA)对19 273例患者和健康体检者血清进行乙型肝炎病毒(HBV)血清学模式的检测,根据乙型肝炎表面抗原(HBsAg)的结果分为阴性组和阳性组。结果 HBsAg阳性组Pre-S1Ag阳性率为61.8%,HBsAg阴性组Pre-S1Ag阳性率为1.4%;HBsAg阳性组的血清学模式主要有7种,以HBsAg(+)、HBeAg(+)、HBcAb(+)和HBsAg(+)、HBeAg(+)模式Pre-S1Ag阳性率最高,占93.3%;HBsAg阴性组的血清学模式主要有7种,Pre-S1Ag阳性率0.1%～6.5%,不同的HBV血清学模式Pre-S1Ag阳性率也有很大差异。结论乙型肝炎病毒Pre-S1Ag的测定在不同乙型肝炎血清学标志物模式中均有重要意义,能对HBV的感染、复制程度做出准确的判断,为临床治疗方案的选择和疗效的观察提供可靠的依据。OBJECTIVE To investigate the relationship between the hepatitis B serological patterns and Pre-SIAg. METHODS The hepatitis B serological patterns of 19273 patients and healthy people were tested by enzyme linked immunosorbent assay （ELISA）, the modes were divided into the negative group and the positive group according to the results of the hepatitis B surface antigen（HBsAg）. RESULTS The Pre-SlAg positive rate of the HBsAg positive group was 61. 8%, the Pre-SlAg positive rate of HBsAg negative group was 1.4%. The serological patterns of HBsAg positive group showed that there were 7 modes, the detection rates of HBsAg（＋）, HBeAg （＋）, HBcAb（＋ ） and HgsAg（＋ ）, HBeAg（＋ ） modes were the highest, accounting for 93.3 % （513/550） ; the serological patterns of HBsAg negative group showed that there were 7 modes, the positive rates of Pre-SIAg ranged from 0. 09% to 6. 46%, the Pre-SIAg positive rate was very different in different HBV serological patterns. CONCLUSION The detection of Pre-SIAg in different HBV serological patterns has important signifi-cance and it is helpful to correctly judge the HBV infection and replication level. It provides reliable rules for the selection of clinical treatment program and the observation of clinical efficacy.
