中医护理干预在鼻咽癌放疗后预防口腔感染中的作用研究

摘　　要：目的分析中医护理干预在鼻咽癌放疗后预防感染中的作用,探讨有效地预防措施。方法 106例鼻咽癌患者随机分为中医护理组和常规护理组,常规护理组在化疗开始后给予常规护理;中医护理组在常规护理的基础上辩证加用中医护理;观察记录两组患者口腔黏膜反应的情况并记录比较。结果第4周中医护理组患者发生口腔感染率86.8%,常规护理组为100.0%;中医护理组患者口腔黏膜反应以Ⅰ、Ⅱ级为主,分别占28.3%、45.3%,而常规护理组患者以Ⅲ、Ⅳ级口腔黏膜反应为主,分别占47.2%、32.1%,且两组4个级别分别比较,差异均有统计学意义(P<0.05)。结论对鼻咽癌放疗后患者进行中医护理干预,可以延迟口腔感染发生,降低感染严重程度。OBJECTIVE To analyze the role of Chinese medicine nursing interventions in the prevention of infections after radiotherapy and explore effective prevention measures. METHODS A total of 106 patients with nasopharyn-geal carcinoma were randomly divided into the TCM care group and the routine care group. The routine care group received conventional care after the start of chemotherapy; the TCM care group was additionally treated with Chinese medicine dialectical care on the basis of the routine care; then the incidence of oral mucositis in the two groups was observed and recorded. RESULTS At the fourth week, the incidence rate of oral infections was 86.8% in the TCM care group, 100.0% in the routine care group; the type Ⅰ oral mucositis and type Ⅱ oral mucositis were dominant in the TCM care group, accounting for 28.3% and 45.3%, respectively; while the type Ⅲ oral mucositis and type Ⅳ oral mucositis dominated in the routine care group , accounting for 47. 2% and 32. 1%, respectively; as compared with the four types of oral mucositis between the two groups, the difference was statistically signifieant（P〈0.05）. CONCLUSION For the nasopharyngeal cancer patients after radiotherapy, the Chinese medicine nursing intervention can postpone the occurrence of oral infections and lower the severity of oral infections.
