形态学检查在早期眼表感染性疾病诊断中的临床价值研究

摘　　要：目的探讨形态学检查在早期眼表感染性疾病诊断中的临床价值,为早期眼表感染性疾病的诊断提供参考。方法共纳入120例可疑眼表感染性疾病患者,均详细记录相关病史,入院后均行形态学检查,并以最终行细菌或真菌培养结果作为金标准评价其对早期眼表感染性疾病的临床诊断价值。结果形态学检查与培养检查细菌结果比较显示,形态学检查对早期眼表感染性疾病的诊断敏感度为90.9%,特异度为85.1%,阳性似然比为0.70,阴性似然比0.96;形态学检查与培养检查真菌结果比较,其敏感度为88.2%,特异度为81.4%,阳性似然比为0.65,阴性似然比0.95。结论形态学检查在早期眼表感染性疾病诊断中的有较好的临床价值,无论对于细菌学感染或真菌性感染均有较高的诊断敏感度及诊断特异度,可以早期给予患者及时的诊断以便及时给予相应的治疗措施,值得临床推广应用。OBJECTIVE To explore the clinical value of the morphological examination in the early diagnosis of ocular surface infectious disease so as to provide basis for the clinical diagnosis of the early ocular surface infectious disease. METHODS A total of 120 cases of patients who were suspected for ocular surface infectious disease were enrolled in the study , the related medical history was recorded in detail, the morphological examination was carried out after the admission , and the clinical value in diagnosis of the ocular surface infectious disease was evaluated by the result of the bacterial or fungal culture read as the gold standard. RESULTS As compared with the result of morphological examination and the result of bacterial Culture, the sensitivity of the morphological ex-amination in the early diagnosis of ocular surface of infectious diseases was 90. 9%, the specificity 85.1%, the positive likelihood ratio 0.70, and the negative likelihood ratio 0.96 ; as compared with the result of the morpho-logical examination and the result of fungal culture, the sensitivity was 88.2%, the specificity 81.4%, the posi-tive likelihood ratio 0.65, and the negative likelihood ratio 0.95. CONCLUSION The morphological examination has good clinical value in the diagnosis of early ocular surface infectious diseases, it has high sensitivity and speci-ficity in the diagnosis of both the bacterial infections and the fungal infections. It is necessary to give the patients the timely diagnosis and the corresponding treatment measures in the early stage, which is worthy to be promoted in the hospital.
