背腧穴埋线对围绝经期抑郁焦虑妇女HAMD评分和血清性激素的影响

摘　　要：目的:观察背腧穴埋线对围绝经期抑郁焦虑患者HAMD评分和性激素的影响,探讨背腧穴埋线治疗围绝经期抑郁焦虑作用机理。方法:将92例患者随机分为埋线组和针刺组,每组46例。埋线组选取肾俞、心俞、肝俞、脾俞、三阴交进行穴位埋线治疗,每周1次,连续埋4次为1个周期,共埋2个周期。针刺组选取与埋线组相同的穴位,常规刺法,隔日1次,每周3次,疗程同埋线组。观察比较治疗前后两组患者的HAMD评分及其对血清促卵泡生成激素(FSH)、促黄体生成激素(LH)、雌二醇(E2)的影响。结果:治疗后埋线组HAMD评分及血清性激素的变化(FSH、LH降低,E2升高)同针刺组相比差异无显著性意义(P>0.05);而埋线组与针刺组自身组内治疗前后HAMD评分及血清性激素的变化(FSH、LH降低,E2升高)比较差异均有显著性意义(P<0.01)。结论:埋线组和针刺组均能有效调节围绝经期抑郁焦虑患者的下丘脑-垂体-性腺轴,并能显著地改善患者的抑郁焦虑症状。因此背腧穴埋线可作为围绝经期抑郁焦虑患者的一种治疗手段。Objective：To observe the influence on HAMD score and sex hormones in patients with perimenopausal depression and anxiety, so as to explore the therapeutic effect on patients with perimenopausal depression and anxiety by catgut implantation at Back- shu points. Methods ：92 patients were randomly divided into the catgut implantation group and acupuncture group,46 cases in each group. The catgut implantation group was treated by catgut implantation at Ganshu （ BL18 ）, Pishu （ BL20）, Xinshu （ BL15 ）, Shenshu （ BL23 ）, Sanyinjiao （ SP6 ）, once a week, continuously buried 4 times as one therapeutic course, and a total of 2 cycle periods. And the acupuncture group selected the same acupoints as catgut implantation group, used routine acupuncture, once every other day,3 times per week, four weeks as one course,2 coures in total. Before and after treatment, HAMD score and changes of contents of serum folliclestimulating hormone （FSH）, luteotropic hormone （LH） and estradiol （ E2 ） were observed. Results ： There was no obvious difference of HAMD score and the changes of sex hormones（ FSH and LH declined, E2 increased）after treatment in two groups （P 〉 0.05）. But treatment HAMD score and the serum sex hormone changes（ FSH and LH declined, E2 increased）in both catgut implantation group and acupuncture group after treatment had significant differences （ P 〈 0. 01 ）. Conclusions ： Catgut implantation group and acupuncture group can effectively regulate the perimenopausal depression and anxiety of patients with hypothal- amus - pituitary - gonadal axis, and have a good effect on the improvement of perimenopansal depression anxiety. So catgut implantation at Back - shu points can be used as a treatment for perimenopausal patients with depression and anxiety.
