降浊合剂治疗糖尿病前期气虚痰浊证临床研究

摘　　要：目的:临床观察中药复方降浊合剂对气虚痰浊型糖尿病患者的临床疗效,探讨其作用机理。方法:154例中医辨证为气虚痰浊型的2型糖尿病前期患者,随机分为两组,A组71人,B组83人,A组在饮食控制及运动锻炼的基础上加用降浊合剂;B组仅用饮食控制及运动锻炼。结果:经2个月治疗,A组患者空腹血糖和空腹胰岛素水平明显降低,空腹胰岛素敏感性指数较治疗前显著升高;B组空腹血糖水平无明显变化,餐后血糖较干预前明显升高,餐后2 h ISI明显降低;A组治疗后TG、LDL、FFA水平较治疗前明显降低。经过1年的随访,A组的糖尿病转化率和血糖达标率在2个月、4个月及12个月时均显著优于B组。结论:中药复方降浊合剂较长期的服用,能有效改善气虚痰浊型糖尿病前期患者的糖代谢紊乱,对其空腹血糖、空腹胰岛素及空腹胰岛素敏感性指数具有显著的改善作用,同时能够改善TG、LDL、FFA水平,并能够有效降低糖尿病前期患者的1年糖尿病转化率。Objective ： To observe the effect of Jiangzhuo Mixture for prediabetes with Qi deficiency and phlegm - turbid syndrome and to study its mechanism. Method： 154 cases of prediabetes with Qi deficiency and phlegm -turbid syndrome were randomized into two groups with 71 patients in group A and 83 in group B. Group A was treated by diet control and exercise added by Jiangzhuo Mixture. Group B was treated by diet control and exercise only. Result： After 2 months＇treatment, in group A, the fasting blood glucose （FBG）and fasting insulin were decreased greatly, the insulin sensi- tivity （ISI）was increased greatly;in group B, fast blood glucose change had no significance, postprandial blood glucose was increased apparently and postprandial ISI was decreased. Triglycerides （TG）, low density lipoprotein cholesterol （LDL） and the free fatty acid（FFA） were decreased after treating in group A. Conversion rate of DM and standard rate of blood glucose in group A were better than that in group B by 2 months ,4 months and 12 months. Conclusion： Jiangzhuo Mixture can relieve glucose metabolism disorder,decrease FBG,TG, LDL and FFA, increase ISI, reduce DM conversion rate with one year for prediabetes with Qi deficiency and phlegm - turbid syndrome.
