不同剂量立普妥联合降脂汤在急性冠脉综合征患者早期应用的疗效评价

摘　　要：目的:研究急性冠脉综合征(ACS)患者早期应用不同剂量立普妥联合降脂汤对血脂、血hsCRP、MMP-9水平的影响,以对其疗效和安全性进行评价。方法:将78例ACS患者随机分为对照组(24例),小剂量立普妥组(26例),大剂量立普妥组(28例),观察用药10 d后各指标的变化情况。结果:治疗10 d后,20 mg/d小剂量立普妥组和40 mg/d大剂量立普妥组血TC、LDL-C、MMP-9和hsCRP的浓度较治疗前显著降低(P﹤0.05),而且40 mg/d立普妥组降低更为显著(P﹤0.05)。TG也降低但是无显著性差异,HDL-C升高但无显著性差异。40 mg/d大剂量立普妥组ALT显著升高(P﹤0.05),CK明显降低(P﹤0.05)。40 mg/d大剂量立普妥组与20 mg/d小剂量立普妥组比较MMP-9浓度降低更显著(P﹤0.05)。对照组前后比较均无显著差异。结论:ACS早期应用立普妥联合降脂汤可以快速降低血脂、抗炎及稳定斑块。40 mg/d立普妥比20 mg/d立普妥能够发挥更强的降低血脂和稳定斑块的作用。Objective：To study the effect of blood fat, hs- CRP, MMP- 9 of acute coronary syndrome（ACS） patients treated with different - dose Lipitor associated with kata - lipoids decoction in early stage. Methods ：78 ACS patients were divided randomly into the control group （n = 24 ） , 20m g/d Lipitor group （n = 26 ）, 40 mg/d Lipitor group（ n = 28 ）, to observe the changes of blood fat, hs - CRP, MMP - 9 before and after treatment of each group. Results ： After 10 d treatment, the concentrations of blood TC, LDL - C, hs - CRP, MMP - 9 were decreased than those before treatment in 20 mg,/d Lipitor group and 40 mg/d Lipitor group （P 〈 0.05 ） , and 40 mg/d Lipitor group was more obvious （P 〈 0.05 ）. The concentration of blood TG and HDL - C changed but there was no significant difference. The. concentration of blood ALT was increased and CK was decreased obviously in 40 mg/d Lipitor group, and the concentration of blood MMP - 9 was decreased more obviously in 40 mg/d Lipitor group than that in 20 mg/d Lipitor group. Conclusion：Lipitor used in early stage can reduce blood fat, anti - inflammatory and stabilize plaque. 40 mg/d Lipitor has better function than that of 20 mg/d Lipitor.
