神经导航下三叉神经感觉根射频术治疗顽固性三叉神经痛

摘　　要：目的探讨神经导航下三叉神经感觉根射频术治疗顽固性三叉神经痛的有效性和安全性。方法回顾分析39例在局部麻醉下行神经导航下三叉神经感觉根射频术的顽固性三叉神经痛患者，观察术前和术后1d、7d、1个月、6个月、12个月、24个月疼痛视觉模拟评分（VAS），统计手术成功率、并发症并计算各时间点治疗的有效率。结果除1例患者因解剖异常外，余38例均获得成功，随访36例术后1d、7d、1个月、6个月、12个月、24个月镇痛治疗有效率分别为88.89%、94.44%、94.44%、91.67%、86.11%、75%，VAS评分治疗前后有统计学意义（P〈0.01）。结论神经导航下三叉神经感觉根射频术治疗顽固性性三叉神经痛具有成功率高、创伤小、并发症少等优点。Objective To explore the effectiveness and safety of treating trigeminal refractory oftrigeminal neuralgia in the sensory root by radio frequency under nerve neuronavigation. Methods 39 cases of trigeminal refractory of trigeminal neuralgia in the sensory root were treated by radio frequency under nerve neuronavigation with local anesthesia, The Visual simulation score （VAS） of pain, the successful rate of operation, complication was recorded and the effective rates were accessed various time points such as preoperation, 1d,7d,6 months, 12 months,24 months respectively. Results 38 cases was successful except for 1 case who had an abnormal anatomy, the patients were followed up and the effective rates for analgesia on time points of ld,7d,6 months, 12 months,24 months were 88.89%~ 94.44%~ 94.44%, 91.67%, 86.11%~ 75%, the VAS was significantly different （ P〈0.01 ） . Conclusions The treatment of trigeminal refractory of trigeminal neuralgia in the sensory root by radio frequency under nerve neuronavigation has the advantages of high successful rate, less injury and complication.
