摘  要:目的探讨封闭负压吸引术(vacuum sealing drainage,VSD)联合皮瓣修复在外伤性小腿骨外露中的应用效果。方法外伤性小腿骨外露患者30例,清创后测量骨外露面积,随机分为试验组和对照组。试验组20例行VSD治疗;对照组10例常规换药。7d后测量2组骨外露面积,然后视情况而定行皮瓣移植。结果 7d后试验组患者骨外露创面均出现新鲜肉芽组织覆盖,骨外露面积明显减小,无感染情况。移植后全部患者皮瓣存活良好,没有发生创面坏死及骨外露复发。对照组中10例患者行皮瓣移植,存活8例。结论 VSD能够有效促进肉芽组织生长,使骨外露面积明显减小,降低感染率,有利于皮瓣的修复和存活。Objective To explore the effect of the application of vacuum sealing drainage combined with flap transplantation in the treatment of traumatic leg bone exposure. Methods The bone exposed area of 30 cases of traumatic leg bone exposure was measured after debridement. The patients were randomly divided into experimental group and control group. Twenty cases of the experimental group received VSD treatment, 10 cases in control group received routine dressing change. The exposed area of bone was measured after 7-day-treatment. And depeuding on the circumstances, flap transplantation was undergone. Results Bone exposure of experimental patients were coverd with fresh granulation tissue and the exposed area significantly reduced without infection after 7 days. Skin flap survived well after transplantation in all patients without wound necrosis and exposed bone recurrence. In the control group, 8 cases in 10 cases of skin flap transplantation survived well. Conclusion VSD on the lower leg bone exposure can effectively promote the growth of granulation tissue, accelerate the repair of wound and reduce the exposure area. VSD also can lower the infection rate and promote the survival of the flaps.
