呼吸过滤器对Supreme喉罩通气患者支气管黏液纤毛运输功能的影响

摘  要:40例美国麻醉医师协会(ASA)Ⅰ~Ⅱ级择期全身麻醉下Supreme喉罩通气患者随机均分为两组。一组未连接呼吸过滤器(A组),另一组连接呼吸过滤器(B组),两组患者分别在机械通气5 min和60 min后测量支气管黏液运输速度(BTV)。在纤维支气管镜引导下,在右主支气管背部黏膜上方通过硬膜外导管注射亚甲蓝,标记染料在0、2、4、6min的移动距离计算BTV值。两组组内比较:A组机械通气60 min后较通气5 min后BTV明显降低(P<0.05)。B组在机械通气5 min和60 min后BTV差异无统计学意义。组间比较:在机械通气5 min后两组BTV差异无统计学意义;在机械通气60 min后B组BTV明显高于A组(P<0.05)。Forty patients with American society of anesthesiologist (ASA)physical status classification I to II were equally randomized into two groups: without breathing filters (group A), with breathing filters (group B). All the forty patients were scheduled for elective surgery in general anesthesia and Supreme laryngeal mask airway. Bronchial mucus transport velocity(BTV) was measured 5 and 60 minutes after mechanical ventilation. An 18-gauge epidural catheter was inserted into right main bronchus, and methylene blue was injected onto the posterior surface of right main bronchus under bronchofibroscope. The travel distances of the methylene blue were determined after 2, 4, and 6 minutes, and BTV was calculated. In A group, BTV measured 60 minutes was significantly lower than that measured 5 minutes after mechanical ventilation ( P 〈 0. 05 ). In B group, BTV had no significant difference between 5 minutes and 60 minutes after mechanical ventilation. There was no significant difference in BTV meas- ured 5 minutes after mechanical ventilation between A and B groups, but BTV measured 60 minutes after mechani- cal ventilation in B group was significantly higher than that in A group ( P 〈 0. 05 ).
