肾筋膜扩张器联合腔内治疗在男性尿道狭窄治疗中的应用(附29例报告)

摘　　要：目的探讨筋膜扩张器在男性尿道狭窄治疗中的临床价值。方法回顾性分析29例用筋膜扩张器治疗的尿道狭窄男性患者的临床资料,探讨筋膜扩张器在尿道狭窄治疗中的应用适应征及影响预后的因素。29例患者,狭窄位于阴茎部尿道8例、球部尿道9例、膜部尿道6例、前列腺部尿道6例。先用输尿管镜探查尿道,在直视下放入斑马导丝,再用筋膜扩张器沿导丝循序扩张尿道。结果 29例患者均用筋膜扩张器成功扩张,未出现并发症。28例获得随访,其中2例出现狭窄复发。结论对男性尿道狭窄用筋膜扩张器行尿道扩张简单、安全、有效、损伤小的方法;减少了并发症,并可为进一步腔内治疗创造条件,但对狭窄段过长,病变严重,尿道情况不明确的患者宜选用其他方法治疗。Objective To probe the clinical application value of the renal sheath dilators on treatment of the urethrastenosis.Methods To retrospectively analysis of the clinical data of 29 cases of male patients treated for urethral stricture by fascia dilator and to discuss the indication and prognostic factors of renal sheath dilators in the treatment of urethrostenosis.Among the 29 patients,there are 8 cases whose stenosis located in the penile urethra,9 cases in the bulbar urethra,6 cases in the membranous urethra,6 cases in the prostatic urethra.At the first,to explore the urethra with ureteroscope,insert zebra wire under direct vision,and then expand the urethra by fascial dilator along the zebra wire sequentially.Results Urethra were all expanded successfully,without complications.28 cases were followed up,two cases of stricture recurrence.Conclusion Dilationofurethraby renal sheath dilators is a simple,safe,effective method with microlesion which reduce complications and create conditions for further intracavitary treatment,but cases who has the long narrow section,with unclear urinary tract situation should use other methods of treatment.
