超声在评价充血性心力衰竭患者心脏非同步化运动中的应用效果

摘　　要：目的探讨超声在评价充血性心力衰竭患者心脏非同步化运动中的应用效果。方法选取浙江省丽水市中心医院2010年2月～2011年2月收治的充血性心力衰竭患者58例,按患者左室射血分数(LVEF)分为3组,LVEF值在34.0%～45.0%者为A组(n=27),25.0%～<34.0%者为B组(n=20),<25.0%者为C组(n=11);按QRS波时限情况分为3组,QRS波时限值在120～130 ms者为A1组(n=22),131～140 ms者为B1组(n=20),>140 ms者为C1组(n=16)。分析不同LVEF、QRS波时限与心脏同步化运动相关指标的关系。结果①A组、B组、C组间左室舒张充盈时间/R-R间期[(43.6±6.7)、(37.4±5.7)、(35.0±4.2)]、左室、右室射血前时间差值(LRVPEI)[(44.3±5.8)、(46.3±6.2)、(55.6±7.4)ms]、SPWMD[(94.5±11.6)、(155.3±22.4)、(189.4±28.4)ms]、Ts1[(49.3±8.9)、(72.4±8.5)、(81.4±11.4)ms]、Ts2[(52.1±8.4)、(73.4±6.7)、(85.5±11.7)ms]比较,差异有统计学意义(均P<0.05);其中,B组、C组左室舒张充盈时间/R-R间期、LRVPEI、SPWMD、Ts1、Ts2与A组比较,差异有统计学意义(P<0.05);B组左室舒张充盈时间/R-R间期、LRVPEI、室间隔和后壁收缩末时间差(SPWMD)、Ts1、Ts2与C组比较,差异有统计学意义(P<0.05)。②A1组、B1组、C1组间左室舒张充盈时间/R-R间期[(44.5±7.8)、(36.3±5.6)、(32.2±9.3)]、LRVPEI[(41.5±5.4)、(48.5±6.7)、(56.8±8.6)ms]、SPWMD[(101.3±16.5)、(142.4±22.7)、(178.4±22.8)ms]、Ts1[(50.2±7.3)、(71.2±6.4)、(80.3±8.9)ms]、Ts2[(50.2±7.3)、(71.2±6.4)、(80.3±8.9)ms]比较,差异有统计学有意义(P<0.05);其中,B1组、C1组左室舒张充盈时间/R-R间期、LRVPEI、SPWMD、Ts1、Ts2与A1组比较,差异有统计学意义(P<0.05);B1组左室舒张充盈时间/R-R间期、LRVPEI、SPWMD、Ts1、Ts2与C1组比较,差异有统计学意义(P<0.05)。③左室舒张充盈时间/R-R间期与QRS波时限、左室舒张末期内径、左室舒张末容积、左室收缩末容积均呈负相关性,差异有统计学有意义(r=-0.12、-0.21、-0.21、-0.41,均Objective To investigate the application of ultrasound in non-synchronized movement in the evaluation of patients with congestive heart failure.Methods 58 patients with congestive heart failure from February 2010 to February 2011 in Central Hospital of Lishui City were selected and divided into 3 groups according to the LVEF.27 cases were found in group A with LVEF 34.0%-45.0%,20 cases were found in group B with LVEF 25.0%-＆lt;34.0%,11 cases were found in group C with LVEF ＆lt;25.0%.The patients were divided into group A1（n = 22） with QRS 120-130 ms,group B1（n = 20） with QRS 131-140 ms,group C1（n = 16） with QRS ＆gt;140 ms.The relevance between the cardiac synchronization motion indicators and LVEF,QRS were analyzed.Results ①The differences of left ventricular diastolic filling time/R-R time [（43.6±6.7）,（37.4±5.7）,（35.0±4.2）],LRVPEI [（44.3±5.8）,（46.3±6.2）,（55.6±7.4） ms],SPWMD [（94.5 ±11.6）,（155.3 ±22.4）,（189.4 ±28.4） ms],Ts1 [（49.3 ±8.9）,（72.4 ±8.5）,（81.4 ±11.4） ms],Ts2 [（52.1 ±8.4）,（73.4±6.7）,（85.5±11.7） ms] in group A,group B,group C were statistically significant（P ＆lt; 0.05）;left ventricular diastolic filling time/R-R time,LRVPEI,SPWMD,Ts1,Ts2 in group B,group C were found had statistically significant differences with group A（P ＆lt; 0.05）;the differences of left ventricular diastolic filling time/R-R time,LRVPEI,SP WMD,Ts1,Ts2 in group B and group C were statistically significant（P ＆lt; 0.05）.②The differences of left ventricular diastolic filling time/R-R time [（44.5 ±7.8）,（36.3±5.6）,（32.2±9.3）],LRVPEI [（41.5±5.4）,（48.5±6.7）,（56.8±8.6） ms],SPWMD [（101.3±16.5）,（142.4±22.7）,（178.4±22.8） ms],Ts1 [（50.2±7.3）,（71.2±6.4）,（80.3±8.9） ms],Ts2 [（50.2±7.3）,（71.2±6.4）,（80.3±8.9） ms] in group A 1,group B 1,group C 1 were statistically significant（P ＆lt; 0.05）;left ventricular diastolic filling time/R-R time,LRVPEI,SPWMD,Ts1,Ts2 i
