浦东新区新报告HIV感染者的送检人群分类及管理依从性分析

摘　　要：目的调查上海市浦东新区新报告的艾滋病病毒(HIV)感染者的送检人群分类情况,对不同送检人群的管理依从性进行分析和比较,找出依从性较差的人群,方便今后有针对性地开展强化管理,提高感染者的随访质量。方法采用横断面调查的方法,对2011年1月1日至2012年6月30日期间,浦东新区所有新报告的HIV感染者的流行病学调查资料和随访资料进行统计描述与分析。结果共收集到新报告的HIV感染者249人,其中男性212人,女性37人,男女性别比例为5.7∶1;感染者主要来自艾滋病自愿咨询检测门诊(VCT)(28.9%)、性病门诊(21.7%)、其他门诊(31.3%)和术前检测(9.6%)。VCT门诊主动检测人群、医疗机构检测人群和非医疗机构检测人群中发现的HIV感染者,接受随访管理依从性较好的比例分别为86.1%、76.9%和50.0%,差异有显著的统计学意义(χ2=6.61,P=0.03)。结论 VCT门诊和医院门诊是浦东新区发现HIV感染者的主要场所,社区吸毒人群和监管场所羁押人群中发现的HIV感染者虽然所占的比例较少,但接受随访管理的依从性较差,应对该人群开展有针对性的强化管理。Objective To investigate the classification of newly reported human immunodeficiency virus（HIV） patients in Pudong New District,Shanghai,and compare their compliance with management,and understand which group of HIV patients should be paid special attention so as to enhance relevant management and improve the quality of follow-up.Methods A cross-sectional survey was conducted to collect the information about social and demographic characteristics and the follow-up records of newly reported HIV patients from January 2011 to June 2012,and statistical description and analysis were performed.Results Totally 249 newly reported HIV patients were enrolled.Of them 212 were males and 37 were females,the gender ratio was 5.7∶1.These HIV patients were mainly detected in voluntary counseling and testing（VCT） clinics（28.9%）,venereal disease outpatient departments（21.7%） and other outpatient departments（31.3%）,as well as via pre-operation examinations（9.6%）.HIV patients showing good compliance with management from VCT clinics,medical institutions and non-medical institutions accounted for 86.1%,76.9% and 50.0%,respectively.The difference was statistically significant（χ2=6.61,P=0.03）.Conclusion VCT clinics and hospital outpatient departments are the main venues of detecting new HIV patients in Pudong New District.Although HIV patients identified among community drug users and drug users in custody account for a low proportion,their compliance with management is comparatively low.Strengthened management should be provided to this population group.
