射频消融迷宫手术治疗长期持续性房颤

摘　　要：目的评价心内直视手术同期应用双机射频消融系统治疗长期持续性房颤的近期疗效。方法 2009年1月至2011年2月,9例心脏手术患者在瓣膜手术或先心矫治手术同期行双极射频消融MazeIII术。患者年龄(59.6±16)岁,平均房颤持续时间5.8年,左心房内径(56.1±5.9)mm。术后随访并评价其疗效。结果本组因左心室破裂死亡1例,射频时平均主动脉阻断时间13.5min。术后即刻、出院时及平均随访9个月时房颤消融成功率分别为100%、75.0%、87.5%。结论心内直视手术同期行双极射频消融MazeIII术治疗瓣膜病或者简单先心合并长期持续性房颤操作上是安全可行的,近期疗效好。Objectives The present study was designed to evaluate the short term effect of modified Cox-Maze III procedure using bipolar radiofrequency ablation（BPRF） on the combination of simultaneous heart surgery and long-standing permanent atrial fibrition（AF）. Methods 9 patients undergo heart surgei-y include 7 mitral valve replacement, 1 mitral valve plasty, 1 case of tricuspid valve plasty with atrial septal defect with permanent AF received RPRF modified Cox-Maze I11 procedure using the Medtronic Cardioblate BP system from. The therapeutic effect was evaluated. Results The average age of .the patient were（59.6＋16）years, average duration of AF were 5.8 years and the average left atrial diameter was （56.1±5.9）mm mm. One patient died because of rupture of left ventricular. The average time for cross clamping of the aorta was 13.5 minutes. The AF was disappeared in all patients immediately after the procedure. Successful restoration of sinus rhythm has been achieved in patients during the follow up of 9 months, while one patient had recurrence of AF. Conclusion The Cox-Maze III procedure with BPRF using the Medtronic Cardioblate BP system is a safe and effective treatment for patients suffering from valve disease combined with permanent AF.
